Job summary
Due to expansion, we
are looking for a pharmacy technician to join our team located at Highbridge
Medical Centre.
We offer full training
and support from our established SHS team of pharmacy technicians and
pharmacists, and you will be given opportunities to develop your chosen career
pathway.
You will assist
with medicines reconciliation and answer medication queries from patients and
community pharmacies, as well as supporting the practice with all aspects of
medication safety. You will liaise with GPs, practice staff, community
pharmacies and secondary care colleagues, and have direct contact with patients
and carers.
You will need
to demonstrate ability to use initiative, have a positive attitude and approach
towards problem solving, and be a good team player.
Both part time
and full time hours considered.

Main duties of the job
To support the practice
pharmacist and clinical teams, including:
- support
the practice pharmacist with medicines reconciliation in accordance with
practice protocols and to update patients medical records and
prescriptions.
- carry
out medication switches, dose optimisation and medicines synchronisation
to ensure safe and cost-effective prescribing
- initiate
repeat prescribing (eRD) for appropriate patients
- provide
advice to GPs, staff and patients in changes to prescribing in order to
support the improvement of prescribing safety, quality and cost
effectiveness
- participate
in medication audits and QOF quality improvement projects
- support
practices to meet QOF indicators
- support
patient safety with high risk drug monitoring and other safety scorecard
indicators
- respond
to medication queries from patients and staff (received via AskMyGP,
phone, e-mail) in a professional manner referring to the appropriate clinician
as necessary
- advise
on the sourcing and safe management of medicines as appropriate
- action
drug alerts
