Job summary

We have an opportunity to appoint another Care Co-ordinator to the West Somerset Primary Care Network to work alongside our expanding team providing support to all of the West Somerset GP surgeries, providing coordination and navigation of care and support across health and care services based at our neighbourhood hub in Minehead.

Care coordinators play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those with long-term conditions. Much of this role will be administrative, reviewing patients needs and helping them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate.

The successful candidate will be caring, dedicated, reliable and enjoy working with a wide range of people. They will have good written and verbal communication skills, computer and digital skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support.

Main duties of the job

Provide coordination and navigation for people and their carers across health and care services, working closely with and referring to social prescribing link workers, health and wellbeing coaches, and other primary care professionals; helping to ensure patients receive a joined-up service and the most appropriate support.

To champion the needs of those most vulnerable to poor health outcomes by working with PCN colleagues to identify and address access barriers to facilitate improvements in access to services, patient experience, and health outcomes.

Work collaboratively with GPs and other primary care professionals within the PCN to proactively identify and manage a caseload, which may include patients with long-term health conditions, or those experiencing health inequalities and where appropriate, refer to other health professionals within the PCN.

Support the coordination and delivery of multidisciplinary teams with the PCN.

Support PCNs in developing communication channels between GPs, people and their families and carers and other agencies including care homes.

Identify unpaid carers and help them access services to support them.

The Care Coordinator will work with the PCN and practice teams supporting them with the NHS England Early Cancer Diagnosis workstream and initiatives.

Triage and process referrals received via the Living Better Outlook inbox and maintain records of referrals and interventions.
