Job summary
Working under the supervision of the responsible pharmacist, carry out the day to day activities connected with the preparation, supply, ordering and receipt of medicines. Will also provide advice to patients on the use of medicines and on general health and lifestyle matters within own capability. To accurately dispense clinically checked prescriptions against the relevant prescription documentation, referring any areas of doubt promptly to the ACT or duty Responsible Pharmacist To contribute to responsibility for the ordering and management of all medicines within KCHFT, working collaboratively together with the ACTs, the other dispensing assistants and under the guidance of the pharmacy manager To identify potential problems in the administration of medicines such as medicine interactions, newly prescribed drugs, physical factors that could affect patient compliance etc. To contribute to the development and maintenance of a high quality and timely outpatient service for patients. To contribute to the safe and accurate issue of completed dispensed prescriptions to patients To contribute to the maintenance of complete and accurate patient medication records for all supplies from the pharmacy To work collaboratively with the ACTs and other dispensing assistants to get the most from pharmacy automation
Main duties of the job
To contribute to the delivery of timely, effective and efficient technical pharmacy services To contribute to the development of, maintenance of and adherence to dispensing Standard Operating Procedures. To ensure prescription records patient medication records represent a complete and accurate record and audit trail of all medicines supplies from the pharmacy To encourage good communication between all pharmacy staff by personal example To promote the extended roles of dispensing assistants and the appropriate use of technology. To contribute to ensuring that medicines are stored in appropriate conditions and that temperature logs are kept. To contribute to ensuring that procedures are in place for the safe disposal of returned medicines from individuals or households and expired pharmacy stock medications. To contribute to keeping the main information services and/or reference sources readily available and up to date. To contribute to the updating of patient information leaflets, posters and if relevant website To contribute to ensuring the safe and accurate issue of completed and checked dispensed prescriptions to patients Handling of Controlled Drugs To contribute to maintaining the day-to-day management, ordering, receiving, storing and recording and auditing of CDs under the direction of the pharmacy manager. To contribute to the development and maintenance of standard operating procedures (SOPs) covering each of the aspects of the safe management of CDs. To contribute to ensuring that the controlled drug register is maintained and the balance is reconciled against the stock of every CD via a regular stock audit Medicines management To contribute to safe and secure medicines practice. This includes medicines selection, procurement, storage, preparation, dispensing, supply and patient education. To monitor and report to the pharmacy manager on adherence of prescribing practice to the hospital formulary To contribute to the maintenance and development of the pharmacy quality management system. To identify and promptly report to the duty Responsible Pharmacist potential problems in the administration of medicines such as medicine interactions, newly prescribed drugs, physical factors that could affect patient safety, compliance etc. Financial To contribute to the purchase, receipt and storage of all medicines from approved suppliers used by the company to a stock level set by the pharmacy manager To contribute to regularly review stock held versus demand in order to achieve a balanced stock holding and to identify in particular products with falling demand and/or short shelf life and regularly report on those to the pharmacy manager To ensure that medicines are handled safely and securely in accordance with SOPs and Health & safety guidelines Confidentiality To respect and maintain the privacy of all information given to and/or held through the performing the role To deal professionally and appropriately with all requests for information covered by the data protection act under guidance from the pharmacy manager Pharmacy Dispenser To ensure that all confidentiality issues that may come to attention directly or indirectly are promptly referred to the pharmacy manager. To maintain the confidentiality of all information relating to the pharmacy business
