Job summary

The primary objectives of the Hospital Stoma Care Nurse role is to provide expert care, advice, and support to stoma patients within the hospital and community. You will assist with the development and management of a patient centred stoma care nursing service at the Bradford Royal Infirmary site and associated hospital clinics and act as a valuable resource for all members of staff at Bradford Hospital.

Main duties of the job

Areas of Responsibility will include:

To provide expert care, advice, and support to stoma patients within the hospital and community.

To assist with the development and management of a patient centred stoma care nursing service at the Bradford Royal Infirmary site and associated hospital clinics.

To act as a valuable resource for all members of staff at Bradford Hospital.

To comply and work within the NMC The Code (2008) and CQC regulations.

To work closely with the community stoma team, ensuring patients receive the highest standards of care in line Salts Medilink working in partnership pathway.

Responsibility for a defined patient caseload, including setting standards of care, assessment of care needs, development, implementation, and evaluation of programmes of care.

Provide education for professionals, patients, and carers.

Provide an integrated and continuing education programme to impact and share knowledge about new developments, research in care and treatment of colorectal conditions.

Work collaboratively with GPs and Medicine Management Teams in implementing clinical patient reviews and assessments as required under the guidance.

Provide education to patients and carers on correct usage and ordering of stoma supplies.

Provide an integrated and continuing education programme to impact and share knowledge about new developments, research in care and treatment of colorectal conditions.

To have extensive knowledge on the diseases, treatments and best nursing practice relating to colorectal diseases and those bladder conditions requiring stoma formation.

Work collaboratively with GPs and Medicine Management Teams in implementing clinical patient reviews and assessments as required under the guidance of the Senior Nurse Manager.

Assist in various stages of company research and development projects, as required.

Be confident to work autonomously without direct supervision.

Be responsible for assessing, planning, implementation and evaluation of evidence-based packages of care for patients, using advanced clinical knowledge and skills.

To support other team members as required in managing specialist caseloads.

Issue and recommend the most appropriate appliance to meet the individual needs of the patient.

Managerial/Operational Responsibilities

Comply with Nurse Team policies and procedures

Provide monthly, timely, written reports to be completed and forwarded as directed to the Nurse Lead for Bradford and Airedale.

Provide written reports as required by the Nurse Lead for Bradford and Airedale regarding issues relating to the provision of care, data collection, annual reports, and service audits.

Introduce and maintain appropriate, efficient systems of referral and review systems of documentation.

Ensure that high standards of nursing care are given and maintained, act when standards are not being maintained.

Attend and participate in nurse meetings and business development as required.

Oversee other areas and cover staff absences, as required.

Support Salts Nurse Team as required with events out of area.

Be proactive in the business planning process to support service development.

Evaluate service development through audit, patient feedback and user involvement as appropriate.

3 Essential Skills / Qualifications required.

Registration on Part 1 of NMC Register.

Relevant post registration qualification at degree or masters level.

3 years post registration experience with relevant experience in stoma care nursing

Demonstrated evidence of personal professional development

Effective communication skills

Membership of RCN

Full UK driving license

Identify individual learning outcomes for self to be discussed and outlined at 6 monthly reviews.

Participate in and present at seminars and study days both internally and externally at local and national events.

Be responsible for achieving CNS competencies.

Update and maintain revalidation portfolio in line with NMC requirements.

Liaise regularly with other specialists within the team to share good practice and promote excellence.

Undertake all mandatory training required for the role.

To produce an Annual Report as directed by the Community Pathway Team Leader.

4 Physical and Social Environment

90% hospital based but flexible as per needs of Salts and BTHFT

5 Staff Development

The Community Pathway Team Leader will have joint responsibility for the training and development of the post holder alongside the hospital team leader. All hospital mandatory training to be arranged by hospital NHS manager/team leader including access to computer systems.

The post holder will have access to Salts training and relevant external study events and be released to attend depending on the needs of the service.
