Job summary

We have an opportunity for a Community CAMHS Navigator to join the organisation on a permanent basis. This is an opportunity to work as part of a charity committed to our vision of enabling everyone in our communities to achieve better mental health. TOG Mind is a fast-paced charity delivering a wide variety of mental health and emotional wellbeing services.

In your role as a Community CAMHS Navigator you will be responsible for supporting children and young people (aged 5-17) transition from statutory mental and emotional wellbeing services, into community support to help aid their recovery, and sustain outcomes following completion of work with CAMHS. You will work with the statutory CAMHS service provided by Pennine Care Foundation Trust, lead provider Early Break and wider Health and Social Care agencies. You will be a team member of our dedicated Children, Young People and Families Directorate, reporting to our Families Manager

You will be providing 1-1 support for a time limited period to CYP within their local communities, in accessible spaces for them. You will provide a range of therapeutic interventions to support their continued self-development and achievement of their self-defined goals.

You will carry a caseload of clients who you provide support to, at times and locations that support access and meet the needs of the client. You will provide a range of therapeutic interventions to support the continued self development and achievement of young peoples self defined goals

You will be familiar with the local community and emotional health and well-being services that are available for children and young people to access.
