This role will require an experienced clinician who has practiced within the care home or frailty environment. JOB ROLES AND RESPONSIBILITIES

Meridian Medical PCN has a number of care homes (residential and nursing) with capacity to provide care for approximately 461 residents. These range in size from smaller units with less than 10 beds to homes with over 30 beds and include some supporting those living with dementia, learning difficulties (LR) and/or complex physical health needs. The post holder will work as part of our local multi-disciplinary team to ensure that all patients in the care setting are treated with dignity and respect. You will build relationships with all care home staff and during working hours act as a point of contact for residents, families and professionals.

The successful candidate will work as part of a multidisciplinary team to manage patients on transfer of care and systems to mitigate hospital admissions. The post holder will complete face to face frailty assessments.
