Job summary

Meridian Medical Primary Care Network (PCN) is made up of four GP Practices: East Lindsey Medical Group and James Street Family Practice in Louth, Marsh Medical Practice in North Somercotes and Tasburgh Lodge in Woodhall Spa.

An exciting opportunity has become available for an enthusiastic and forward-thinking Occupational Therapy Care Coordinator to join the Meridian Medical PCN in a new role that will be developed as it progresses. This role will work closely with the Senior Occupational Therapists, but will also work within our Primary Care Community Team, comprising of GPs, Practice Managers and their teams, Neighbourhood Team Lead and community staff, Clinical Pharmacists, First Contact Physiotherapists, Care Co-Ordinators, Frailty Nurse and Social Prescribing Link Workers

Main duties of the job

The position will require flexible working between the surgeries.

The OT team work with the General Practitioners (GPs) providing proactive support to help people to manage their health and physical abilities and to remain as independent and as active as they are able and want to be.

The post holder will work alongside our PCN Occupational Therapist, supporting them with delegated lower-level equipment assessments/follow up home visits. This post will also require some basic administrative tasks to support the efficient and effective delivery of the OT service but will also include minute taking at Meridian Medical PCN Board Meetings.

The Occupational Therapy Care Coordinator will assist in the coordination and the delivery of occupational therapy services to patients. This position requires excellent organisational and communication skills, as well as an understanding of occupational therapy principles and practices.

The Occupational Therapy Care Coordinator with collaborate with patients, other OT and healthcare professionals to ensure the efficient and effective delivery of the service.

Supervision, professional development, and training opportunities will be available to support this role.

About us

Meridian Medical PCN is made up of the two practices situated in the town centre of Louth, consisting of East Lindsey Medical Group and James Street Family Practice, together with Tasburgh Lodge Surgery in Woodhall Spa and Marsh Medical Practice in North Somercotes. The PCN serves a population of approximately 44,000 patients. Staff working in the PCN are employed by Lincolnshire & District Medical Services Ltd on behalf of the PCN.

LADMS is a federation of GPs delivering primary care medical services in Lincolnshire, predominantly in the East of the county. It's main contracts included GP Extended Access services, INR and anti-coagulation, Section 136, the Covid Vaccination programme and the Enhanced Health in Care Home Framework for Meridian Medical PCN.

Close working relationships exist between the PCN and LADMS

JOB RESPONSIBILITIES:

To act as the central point of contact for practices, patients and their families.

To coordinate and schedule appointments to ensure timely comprehensive care, after referrals have been triaged by the Occupational Therapist.

To conduct basic assessments in the persons, own home, care home or clinic setting under the supervision and guidance of the Occupational Therapist, including gathering medical history, evaluating functional abilities and identifying follow up/further assessment from the Occupational Therapist if required.

Assist the Occupational Therapist in ordering and managing equipment and resources as necessary.

Ensure the proper management of patient records, ensuring all progress notes, treatment plans, discharge summaries and any correspondence are documented accurately and in a timely manner, filed appropriately, maintaining confidentiality and privacy of patient information.

Have a good working knowledge of all practice specific IT systems and general IT systems (Word, Excel, System one etc.)

Facilitate effective communication and collaboration among the healthcare teams, including physicians, nurses, and other allied health professionals. Share relevant information and updates to ensure coordinated and comprehensive patient care throughout all of the GP Surgeries.

Provide education and support to patients, families and caregivers regarding occupational therapy services, treatment and intervention options, and community resources. Offer guidance on home exercise programmes and adaptive equipment to promote independence and enhance patient safety and outcomes.

Ensure that identified patients receive the right level of help at the right time and help them to experience a joined-up service by liaising with relevant organisations.

Deal with general enquiries made by telephone or in writing and ensure that all patients are always treated with dignity, respect and consideration.

Make onward referrals to PCN services such as social prescribing. Ensure good working relationships with these services are formed.

Attend relevant training, conferences, workshops, and continuing education programmes to expand and develop knowledge.

To contribute to improvements in service delivery.

To participate in the organisation and coordination of proactive educational/wellbeing events across the geographical area covered by the GP Practices.

To carry out basic administrative tasks to support the efficient and effective delivery of the OT service.

To attend meetings and complete minute taking at James Street Family GP Practice when required.
