Main duties of the job

The post holder is a Care Coordinator, who acts within their professional boundaries, working alongside the Practice multi-disciplinary team and within the Primary Care Networks. They work for the benefit of patients, providing and maintaining a high standard of care for patients health needs. The post holder will be an integral part of the General Practice team, building relationships with other community providers, community services and working closely in partnership with other staff within the PCN and practices other Social Prescribing Link Workers.

Job responsibilities

Key Responsibilities:

The Care Coordinator responsibilities include but are not limited to the following:

  • To work with the GPs and other primary care professionals to identify and manage a caseload of patients.
  • Identify, whererequired and as appropriate,patient who need to be referredback to other health professionals within the PCN.
  • Providecoordination and navigation for people and theircarersacross health and care services, working closely with social prescribing link workers, health wellbeing coaches and other primary care professionals
  • To support patients to utilise decision aids, help create single personalised care and support plans, in line with best practice.
  • To help people to manage their needs through answering queries, making, and managing appointmentsand ensuring that people have good quality written or verbal information to help them make choices about their care.
  • Supporting the smooth operation of patient care pathways by utilising AI technology to streamline coding, workflow management, and communication processes. This role involves managing and processing patient letters, updating clinical systems accurately, and ensuring timely follow-up actions.
  • Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing.
  • Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to shared decision-making conversations.
  • Provide the interface between primary care and other services, helping patients with the transition between services.
  • Ensures awareness of statutory and local clinical protection procedures, including systems of referral.
  • Attend and participate in any practice meetings when required
  • Any other delegated duties appropriate to the post
