The PCN Manager will support the PCN to develop and implement service improvement
and transformation work, aligned to local and national priorities. This
includes building on current primary care provision, and working proactively
with the wider network to ensure we can offer our local population coordinated
and more integrated healthcare. The role entails building effective working
relationships, working closely with the Clinical Director, engaging
stakeholders across different organisations and disciplines, coordinating
work-streams, collating and monitoring risks and issues, building consensus
across the six practices of NN4.
The PCN Manager will coordinate the successful delivery
of all contractual requirements, translate national and local policy into
meaningful changes, and will support the network to achieve improved outcomes
for our local population. They will also liaise with the PCN Managers of other
PCNs, both within North Norfolk, and across the ICS.
Key Responsibilities:
The PCN
Manager & Digital and Transformation Lead responsibilities include but are
not limited to the following:
To manage compliance with the Network DES
specification on behalf of the NN4 PCN member practices. To include:
o
Achieving maximum achievement to meet Investment
and Impact Fund (IFF) targets and other network-related achievement schemes.
o Produce performance and quality reports within the
Network
Coordinate and report on Quality and Outcomes
Framework (QOF)
Quality Improvement (QI) projects across the PCN.
Work with the Clinical Director and Board to
develop and deliver detailed workforce plans, aligned to PCN priorities, and
reflecting opportunities within the Additional Roles Reimbursement Scheme
(ARRS).
Work with the Clinical Director and Board to develop and deliver a
continuous improvement approach to digital accessibility for our patients and
workforce, including supporting national and local initiatives.
Managing the staff performing ARRS-funded roles
deployed across the PCN to ensure satisfactory leadership and clinical
supervision is in place.
Support
recruitment, induction and training of new PCN staff where required
Work closely with other service providers
engaged to deliver services under the ARRS scheme.
To support the Clinical Director and associated decision-making
at formal PCN and informal meetings.
Manage PCN-related projects.
Represent the PCN in cross organisational
meetings when agreed e.g. locality/Place meetings/ICS-wide meetings
Enable and encourage the adoption of new technology and other
initiatives to improve the care offer and encourage PCN staff to work more
effectively to support the sustainability of general practice services.
Facilitate the collaboration between practices within the PCN to
support the delivery of care to each others patients and learn to drive
continuous improvement.
Work together with the PCNs Practice Managers
to support the delivery of PCN vaccination programmes, ensuring delivery of
NHSE targets.
Use data, and improve data quality, to support decision making that
includes (but is not limited to) demand, capacity, utilisation data and staff
and patient experience.
Support population health management, improvements in care quality and
PCN operational efficiency.
