About The Collaborative Care Team
PMLs Collaborative Care Team (CCT) supports seventeen General Practices located within West Northamptonshire, providing proactive assessment and support to people with longterm and complex conditions and their Carers. We undertake holistic assessment within the home environment, identifying needs and priorities with patients and their carers to improve wellbeing, maintain independence with an overarching aim to prevent unplanned admissions to hospital.
The Team currently comprises two Registered Nurses (RNs), an Occupational Therapist (OT), two Assistant Practitioners (AP), and a Social Prescribing Link Worker. The CCT operates Monday to Friday, 08:30 to 16:30 excluding Bank Holidays

Main duties of the job
To provide daily coordination of the CCT service delivery under the direction of the Clinical Lead, acting as a central point of contact to ensure appropriate support is made available to the team, patients and their carers.
To regularly liaise with the range of multidisciplinary professionals and colleagues involved in a persons care, facilitating a coordinated approach and ensuring everyone is kept up to date so that any issues or concerns can be appropriately addressed and supported. This includes actively participating in multidisciplinary team meetings within the CCT, PCNs and Practices.
To provide administrative coordination for the CCT service delivery supporting PML Governance, Operational and performance compliance.
To develop expertise in the administrative functions of Clinical systems, specifically Systemone and EMIS, including problem solving, staff support, access, reporting and contingency.
To keep accurate and up-to-date records of contacts, appropriately using the CCT clinical system, GP and other records systems relevant to the role, adhering to information governance and data protection legislation.
To support the Clinical Lead to ensure that PML/CCT remains safe and effective, including using audits and learning from significant events and complaints including the collation of evidence for CQC compliance.
