Job summary
We have an exciting vacancy for experienced Practice Nurses to join our bank working across our Extended Primary Integrated Care Services (EPiC).
The post holder will be an experienced Practice Nurse who can work independentlyoutside of core hours to support Enhanced Access Services ensuring to deliver the highest standards of care for all patients accessing our services.
Responsibilities will focus on work in the evenings and weekends to provide practice nurse services such as cytology, chronic disease management and immunisations.
Main duties of the job
Main duties will include;
- To undertake nursing and therapy tasks including cytology, chronic disease management, health checks, vaccinations, assessment of risk factors, and lifestyle advice.
- Manage the care of patients with long term conditions. Assess, plan, implement and evaluate treatment plans for patients with long term conditions (or at risk of developing one) that meet identified needs and promote health and well-being.

The Extended Primary Integrated Care (EPiC) Team is ran by Salford Primary Care Together to bring together primary and urgent care services through a central hub so that patients can be directed to most appropriate level of care they require.
