Job summary

Salford Primary Care Together Community Interest Company (CIC) is looking for experienced Primary Care Digital First Facilitators to join our dedicated team within Salford on a full-time basis, 37.5 hours a week. This role will be based at Salford Primary Care Togethers’ Head Office (2 City Approach, Eccles) and across the other sites as needed within Salford’s 5 Primary Care Neighbourhoods (Lower Broughton, Eccles & Irlam, Ordsall & Claremont, Walkden & Little Hulton, and Swinton).

Main duties of the job

We are looking for someone who is dynamic and conscientious to join our Digital Facilitation Team for primary care, as part of the Greater Manchester digital first primary care program. The digital first primary care program will work across all Salford localities.

This role holds a significant amount of personal responsibility and accountability: You will need to be someone who can self-motivate, problem-solve, and manage your own workload and priorities. You will possess excellent interpersonal and communication skills to be able to build relationships with not only the GP Practices that you work with, but with all other Greater Manchester and general practice digital first programs and projects that you support with.

You will be joining a new team of Primary Care Digital Facilitators and will receive supportive training in helping you develop service improvement and digital transformation skills, as well as primary care IT system training.

KEY RESPONSIBILITIES

· To co-ordinate the use of GP clinical systems and digital technologies within primary care, in order to support improvements to the health of the local population and to support the general management of patients and practices. This will involve supporting neighbourhoods, standardising data recording, and working processes as well as facilitating training and behaviour change with regards to Information Management and Technology within the neighbourhoods.

· To work closely with line managers in order to deliver organisation targets in an efficient and accurate way.

· Maintain excellent communication with all staff, patients, and all stakeholders, ensuring all organisational and practice information is accurate and up to date for programmes and projects.

· Work directly with the Business Intelligence teams to support and report on successful delivery and performance objectives.

· To work autonomously and refer to Informatics manager as necessary.

· To communicate effectively at all levels and multidisciplinary Primary Health Care Teams and to foster and promote effective lines of communication within GP practices.

· Leads the delivery of projects and work packages to meet business needs and deliver agreed benefits by effectively utilising the necessary resources and skills.

There is an expectation for you attend the office as required as well as travel in the area of your locality, this includes across into other areas of Greater Manchester as required, dependent on the programme of work.

The successful candidates will be enrolled on to a mixture of face-to-face training and then, quarterly training will mainly be delivered via Microsoft teams to support skills improvement.

In addition, the Digital First Facilitators will be enrolled into our new digital first academy where they will be able to receive training and support with skills to enable them to be effective facilitators and develop new transferable skills that can be captured and used in this role and help the facilitators in their career journey in the NHS.
