As part of our multi-disciplinary team, you will provide autonomous clinical assessment, examination, and management of minor illness presentations, using both telephone and face-to-face consultations. Working alongside GPs and other professionals, developing advanced clinical skills and contributing to safe, effective, and standardised care.

Main duties of the job
- You
will be a RN, Paramedic, or Allied Health Professional with
the skills to deliver autonomous care across both Extended Access (in-hours)
and Out of Hours (OOH) services. Your role will ensure patients receive timely,
safe, and effective treatment through a combination of telephone consultations
and face-to-face contact.
- You
will demonstrate excellent clinical assessment and decision-making skills, a
passion for patient-centred care, and highly developed communication abilities.
- Assess,
diagnose, treat, and signpost patients appropriately, considering complex needs
and co-morbidities, including frailty.
- Provide
care for minor illness and injury presentations across both in-hours and OOH
settings.
- Work
collaboratively within a multi-disciplinary team (MDT), including GPs and other
healthcare professionals, to maintain continuity and quality of care.
- Respond
flexibly to service demands, adapting to different care settings and patient
needs across Extended Access and OOH environments.
- Maintain
high standards of clinical practice, ensuring safe, effective, and standardised
care for all patients.
- Contribute
to the development of advanced clinical skills in minor illness and injury,
supported by mentorship and training opportunities.
