Practice Nurse

Job summary

CBC Workforce Solutions are looking for experienced Practice Nurses to join their Flexible Workforce Hub on a sessional basis. The Flexible Workforce Hub provides short-term, planned and unplanned workforce support to our GP practices throughout Gateshead.

We are a not-for-profit company with a wide and diverse portfolio of services that we deliver as an NHS provider.

We are looking for an enthusiastic, friendly, outgoing and approachable person, who is a skilled and experienced Practice Nurse. As a Practice Nurse, you will work on a sessional basis, during practice opening hours. You will be a registered nurse with the skills to autonomously assess and treat patients within General Practice. Telephone consultations and face to face contact. You will need to have excellent clinical assessment and decision-making skills along with a passion to provide patient centred care and have highly developed communication skills. Working knowledge of EMIS is essential.

Main duties of the job

Duties include, clinical care interventions, medications, assessment, wound care and management, cervical screening, ear care, vaccines, pre-travel health, prescribed therapies. Health promotion, alcohol screening, smoking cessation, healthy diet, physical activity, weight management, sexual health investigation and contraception and family planning.

Long term conditions screening, diagnosis, ongoing management, and referral to other agencies in line with NICE guidance and Quality and Outcome Framework indicators and also to develop and follow referral pathways.

Risk stratification and care planning. Quality assurance and service support and to be able to recognise opportunities to reduce inequalities and unwarranted variation. You must be able to handle sensitive and confidential information.

Ensuring the privacy, dignity and safety of individuals is maintained at all times, using clinical IT systems, and maintaining accurate patient records. Also, recognising and reporting issues relating to safeguarding vulnerable children and adults.
