Job summary
We
have an exciting opportunity for diabetes clinical pharmacists to join the
Morph Clinical Services team to deliver remote clinical diabetic reviews in primary care. This is a flexible, remote role that offers variety and
diverse workstreams. This job role is open to full & part time candidates
who meet the job requirements- please clearly specify which option you would
prefer. The post would suit a dynamic and enthusiastic person with a broad
level of clinical expertise and with clinical experience in diabetes.
For this job role we are looking for someone
with excellent diabetes management knowledge and experience.
Main duties of the job
Ideal candidates should be able to use clinical expertise to provide evidence based
prescribing recommendations and advice to ensure effective treatment
optimisation in relation to diabetes. Candidates should also have a broad range of clinical knowledge of
management of long-term conditions, be highly organised and work well on their own initiative. Candidates should be up to date with current best practice and be able
to demonstrate interest in continued professional development. Candidates should
be aware of professional, local and national guidelines that will affect their practice.

therapy review services to GP surgeries supporting improved clinical outcomes
and medicines optimisations.
Project dependant, we can support both in-hours and out of hours working.
Job description
Job responsibilities
- Reviewing type 2 diabetic patients holistically with single or multiple medical problems where medicine optimisation is required.
- Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e. medicines optimisation). Make appropriate recommendations to GPs for medicine improvement.
- Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacist, nurses and/or GP on prescribing and monitoring in relation to diabetes.
- Answers relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines.
- Advise on follow up for patients to monitor the effect of any changes
- Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period e.g. pathology results, common/minor ailments, acute conditions, long term condition reviews etc.
- Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches
