Onsite IP Clinical Pharmacist (REF: TPN-CP-HYDE)

Job summary

An exciting opportunity has arisen for clinically focused, highly motivated PCN Clinical Pharmacists who wish to expand their experience by working on-site in a General Practices within the Hyde PCN area.

You will be supported by a national network of pharmacists and technicians who are undertaking the same journey remotely and working on-site.

You will be enrolled on an 18-month national training program, including attainment of an independent prescribing qualification (if applicable). You will also be supported by a national network of pharmacists who are undertaking the same journey.

What we require -

• Previous primary care experience
• Experience with EMIS or System 1 is essential (Training can be provided)
• Desirable to already be on CPPE Pathway or willing to partake
• Able to commute to and round Hyde area
• Independent prescribing qualification is required for this role

Main duties of the job

In recognition of the important role clinical pharmacists can play in general practice we are seeking a professional pharmacist who can support the GP practice with general day to day workstreams using Emis or System 1, which may include the below:

• Resolve medication-related issues
• Helping to utilise skill mix
• Improve patient outcomes
• Ensure better access to healthcare
• Support practice staff with prescription and medication queries
• Medicines reconciliation
• Provide expertise in clinical medicines advice
• Providing clinical leadership on medicines optimisation and quality improvement
