Job summary

We have an exciting new opportunity for a Digital and Transformation Lead within Bromley by Bow Health & Stepney Health CIC and Healthy Island Partnership, Tower Hamlets.

This new role will lead transformation work across two PCNs and use Digital where appropriate to enable the change and maximise the use of the tools available. The Digital and Transformation Lead will work with clinical and operational teams within two Primary Care Networks (PCNs) to improve patient access, support the integration of technological solutions to help practice teams manage workload, improve staff and patient satisfaction, and the efficiency and sustainability of the PCN and Practice services.

You will be working on local and national priority areas in a fast paced moving environment.

Main duties of the job

What Youll Do

  • Build relationships across the PCNs (clinical and non-clinical) to develop consensus for improvements and transformation

  • Improve adoption and/or optimisation of new and existing digital technologies to enhance patient access and experience

  • Identify opportunities to improve the efficiency of clinical and non-clinical process

  • Identify opportunities for continuous improvement to support practices and PCNs manage workload

  • Support and improve the digital maturity of both PCNs

  • Use data and insight to improve population health management
