setting providing holistic, person-centred care in a multi professional team?
Tower Hamlets GP Care Group CIC in partnership with our local GP employers
are delighted to facilitate either a 2 year or 12m fixed term training post. You
will be employed by a local General Practice and study at City St Georges,
University of London for a Post Graduate Diploma (PG Dip) in Practice
Nursing. Alongside this you will be supported by our pioneering Nursing
Development Open Doors clinical tutors. Recruitment will be via Tower
Hamlets GP Care Group and GP practices
Our innovative programme has been running continuously since
2008 and we are proud to say many of the Open Doors nurses have continued
working within Tower Hamlets and gone onto become Lead Nurses, ANPs and
Educators.

Main duties of the job
Being a GPN trainee within Tower
Hamlets, you will be employed and contracted by a GP practice and work with the
multi-disciplinary Practice Team. The
37.5-hour week is divided into clinical sessions and academic study. This is
arranged flexibly to meet the needs of the practice and link with the educational
component of the programme. The university study is offered over 24 or 12
months. It is recommended that the study is taken over 2 years, especially if
the applicant is newly qualified to allow consolidation of clinical skills at
the same time as studying at masters level.

impact the public health of patients and residents in Tower Hamlets via immunisation
programmes for adults and children, national screening programmes, health
promotion.

Providing
education and holistic self-management support for patients with complex and long-term
conditions, (for example diabetes, asthma, COPD, Hypertension, Chronic Kidney
Disease) including consideration of their mental health and social
circumstances, enabling them to stay well and out of hospital.

families and carers who are caring for those with mental or physical problems
and distress.

Clinical
1. Understanding and delivery of national immunisation programmes for both adults and children
2. Understanding and delivery of national screening programmes eg cervical screening,
3. Assessment, planning and support for patients to self-manage long term conditions such as Diabetes Mellitus, Hypertension, Asthma, Chronic Obstructive Pulmonary Disease, Hyertension, Chronic Kidney Disease, Heart Failure and Cardivasular Disease.
4. Provide NHS agreed travel advice and vaccinate appropriately
5. Assess and care for patients presenting with uncomplicated wounds, including removal of sutures and clips
6. Genearal Practice Nursing skills eg urinalysis, peak flow, phlebotomy, NHS Health Checks, ECGs, Spirometry
7. Support and advise patients requesting information relating to reproductive and sexual health
Communication
1. Communicate effectively within the MDT teams, patients and carers both verbally and written
2. Consultation skills to support patients to make evidence based decisions and joint care
3. Anticipate barriers to communication and take action to improve communication
4. Act as an advocate when representing the patients and colleagues viewpoints to others.
Administration

- To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice
- To maintain own continuous professional development (CPD) and ensure CPD is kept up-to-date through annual appraisal in line with the requirements of the Practice and The Code
- Emergency Equipment maintenance and audit
