Job summary
With the continued growth and innovation in Primary Care Networks (PCNs) across Tower Hamlets, PCN 6 Bromley by Bow & Stepney Health CIC and PCN 5 Bow Health Network CIC are seeking two part-time highly motivated and proactive Same Day Access Administrators to join our expanding General Practice teams.
This is a pivotal role aimed at ensuring patients that should be treated in Primary Care receive timely, coordinated, and high-quality care. You will be supporting the PCNs and their Practices to deliver a Same Day Access Pilot Monday to Friday.
This pilot will involve redirecting patients from A&E/UTC and 111 into PCN clinics at different locations across the PCNs. This includes triaging patients, actioning GP-assigned tasks, and contributing to service monitoring and improvement.
This is a fixed term role until 31 March 2026 with the potential to be extended. Hours are 19 hours per week.

Main duties of the job
Key Responsibilities:
- Act as a single point of contact for staff and service users accessing same-day care.
- Support triage and booking of patients into GP practice and 111 slots, as well as appropriate A&E redirects.
- Coordinate the delivery of care plans, working closely with clinicians and allied health professionals.
- Respond to patient queries, manage appointment scheduling, and ensure smooth service access.
- Process healthcare referrals promptly and within set timescales.
- Support the collection and coordination of data for monthly, quarterly, and annual dashboards and performance reports.
- Analyse and interpret service data, communicating outcomes clearly to support decision-making.
- Maintain accurate and up-to-date patient records in line with governance standards.
- Participate in relevant training and contribute to ongoing service development through continuous professional learning.
