Job summary

We have an exciting opportunity within the GP Care Group (GPCG). We seek Healthcare Assistant - Physical Health for our GPCG Extended Access Hub.

As a member of the GPCG Extended Access Hub team, your primary aim will be to provide physical health checks to a defined group of patients at their workplaces in Tower Hamlets. The workplace Health Check will encompass assessments of smoking status, height/weight/BMI, alcohol risk status, blood pressure check, and type 2 diabetes risk score.

This is a fixed term contract for an initial 12 months and you will be based in different sites in Tower Hamlets.

Main duties of the job

Key Responsibilities:

  • Assess individuals cardiovascular disease risk scores at workplaces.
  • Communicate risk scores clearly, using every day, jargon-free language to ensure individuals understand their level of risk.
  • Deliver very brief advice, brief advice, or extended brief interventions to support individual behaviour change.
  • Frame risk messages in a way that motivates individuals to consider changes to reduce their risk.
  • Allocate adequate time to deliver assessments, share, and discuss results with individuals.
  • Provide individualised written information booklets that include full results, bespoke advice on identified risks, and self-referral information for behavioural interventions.
  • Refer or signpost individuals to PCC, NHS, and community services, such as stop smoking services.
  • Advise individuals at high risk to schedule follow-up appointments at their GP practice.
  • Support Commissioners in evaluating the effectiveness and cost-effectiveness of NHS Health Checks in Tower Hamlets.
