The GP Care Group in partnership with Tower Hamlets Local authority are delighted to announce new exciting opportunities having been awarded one of the 5 national Start for Life Health Visiting Workforce Pilot sites. Our pilot programme covers 4 key domains in a child's life journey from conception to 1001 days, with a focus on positive attachment, deliver immunisations, trusted relationships, mental health support, as well targeted communication development, and
having dedicated social prescribing for this age range.
The Speech and Language Pathway Lead is a key member of the Health Visiting Workforce Leadership team and will be responsible for the day-to-day leadership
and management of a designated team of the Communication Development enhanced
pathway team.
This commissioned service pilot is fixed term until March 2025 as
a pilot programme. It presents a unique opportunity for individuals to develop
their skills, whether through a secondment option from their current role or by
stepping into a brand-new position. You'll be part of a dedicated team focused
on making a tangible difference in the lives of parents and babies in Tower
hamlets.

Main duties of the job
- To deliver effective strategic
and operational
and clinical delivery in line with expectation of their Job Description and
model of practice within the workforce Pilot.
- To support through practice the practice the development of the pilot working in line with national policies and locally identified health needs within defined role.
- To support delivery of service transformation in line with role requirements and development of integrated clinical care pathways in response to latest evidence-based practice.
- To work with a range of processes and systems, across the service to support the effective running of the service and clinical care pathways.
- To work within a culture in which innovative ideas can flourish and provide vision and inspiration to enable staff to deliver support effectively.
- To always act as a positive role model and advocate both for individual staff and the Tower Hamlet GP Care Group.

Job description
Job responsibilities
- The post holder will achieve this by utilising best available evidence to improve the health and wellbeing of children and young people especially those from disadvantaged groups.
- The post holder is responsible for the day-to-day leadership the skill mix team within a defined workforce pilot area to develop and deliver universal and early intervention using an integrated model of working across professional and organisational boundaries, within the Family Hub structure.
- The post holder will also be responsible for the safe implementation and delivery of the enhanced pathway to enable delivery of enhanced Speech and Language screening, providing clinical expertise within the Multi-disciplinary team and training provision across the team, Family Hubs, and within the Universal 0-19 team.
- The post holder provides clinical support and supervises junior speech and language therapists and / or bilingual speech and language therapy (SLT) co-workers involved in providing a service to preschool / Early Years Foundation Stage (EYFS) children managed by the Early Years Team. S/He coordinates the delivery of the service in liaison with the Lead SLT for Early Years and ASD.
- In partnership
with the Targeted 0-5 SLT team develop a person-centred and evidence-based
pathway for children 0-2 years in line with the Start for Life Work force
pathway offer for social communication difficulties (including children with
autism and suspected autism) , and their families in co-operation with other
partners in Tower Hamlets .
- Ensure that the
pathway is providing effective and efficient targeted-level support to other
professionals so that families have good access to high-quality intervention at
the time they need it.
- In partnership
with the Targeted 0-5 SLT team develop and lead a specialist-level pathway for
children 0-2 with social communication difficulties that uses the expertise
from speech and language therapists, occupational therapists, therapy
co-workers and others to deliver the best outcomes possible for families
without duplicating effort.
- Alongside the
current speech and language Team Lead for the under 5s pathways, ensure that
families of children between the ages of 0-2, 3 and 5 have access to speech and
language therapy expertise and advice.
- Ensure that the
latest evidence is used in developing and improving pathways
- Report monthly to
commissioners on the progress and improvements in service created as an outcome
of the two-year investment, making sure that the service has effective systems
to gather the evidence needed for this reporting.
- Ensure the participation and inclusion of parents and carers at all
levels of decision-making, promoting consultation and involvement in various
forums.
