We are delighted to announce this exciting opportunity for a full-time post working with the Childrens 0-19 universal 0-19 team as the lead practitioner for marginalised communities in Waltham Forest
This is an exciting opportunity for

The post holder will take a proactive
lead in working collaboratively with the local Children and Family
centres/Family Hubs and the wider childrens workforce inclusive of General
Practice, Public Heath, Early Years/Schools etc to provide integrated and planned
care for vulnerable groups of children and their families. You will have a
designated caseload, to include asylum seekers/homeless hotel placement, and
very hard to reach children and young people working with families with children 0-19 years.
Main duties of the job
You will be working in the Universal Childrens Service
0-19 team, and in partnership with Children and Family Centres/Family Hubs,
system partners to assess and develop plans to address the health needs of
children under 19 years (and up to 25 years for children/Young people who are
SEND) and their families, who are living in temporary /asylum accommodation and
/or meet the criteria for Marginalised communities due to non-engagement,
poor service access.
