Job summary

Tower Hamlets GP Care Group is a federation of 30+ General Practices which cares for more than 312,000 people. You will be responsible for the day-to-day administration of the PCN with tasks including but not limited to the scheduling of regular meetings, MDTs, agenda setting, minute taking and working with EMIS Web.

Network Five is a Primary Care Network (PCN) based in Bow, Tower Hamlets. It serves a population of approximately 35,000 patients and has five GP Practices. The PCN has various integrated ARRS roles; clinical and non-clinical team, who work together to deliver enhanced care to the Network’s population.

In addition to work commissioned via the national Network DES, the Network also co-ordinates local enhanced services for its member practices through the Network Improvement Scheme.

Established in 2010, Network Five is one of the oldest Primary Care Networks in the country and has a history of delivering excellent care to its patients. Fuelled through the additional funding available via the national DES, it is currently undergoing radical expansion into new areas including direct patient care. Now is a very exciting time to get involved.

Main duties of the job

The role involves working very closely with the practices and the multidisciplinary team within the PCN. The role is pivotal in ensuring all patients receive the best possible care and service. The Care Coordinator’s role will support the clinical director and member practices in coordinating all key activity including access to services, advice, and information. Ensuring health and care planning is timely, efficient, and patient-centered. The role will include supporting digital initiatives and includes responsibilities for the co-ordination of the patient’s journey through primary care. You will also work closely with the Network Coordinator on administrative tasks. You will need to develop close working relationships with the PCN’s constituent practices and be able to provide them with support ad-hoc support relating to NISes, finances and more. You will also be expected to assist with liaising with external organisations.

  • Higher education level e.g. Level II Diploma in Health and Social Care or equivalent level of training and experience.
  • Evidence of consistent pattern of learning from education, training and experience.

Experience

  • Experience in Primary Care or Social Care.

Desirable

  • Experience and knowledge of working with communities within Tower Hamlets.
  • Experience of developing relationships with a wide variety of people.

Knowledge and skills

Essential

  • Understanding of human needs: physical, emotional and social.
  • Ability to communicate confidently both with clinical and non-clinical staff.
  • Good time management and prioritisation skills.
  • Ability to work on own initiative.
  • Adaptability and a flexible positive approach.
  • Willingness to undertake further learning and training.
  • Demonstrate commitment to team working.
  • Good IT skills.
  • Fitness to do the job within manual-handling policy.
