Minehead Medical Centre is looking for an experienced Advanced Clinical Practitioner to join our multi-disciplinary team, to help us to transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries.

What youll work on
- Delivering safe, effective, and compliant care to patients across the practice
- Making autonomous clinical decisions in assessment, diagnosis, and treatment
- Managing planned and urgent primary care presentations, including minor illness
- Working closely within a multidisciplinary team to ensure patients receive the right care at the right time
- Contributing to clinical governance and quality improvement, alongside the Lead Nurse
- Providing clinical leadership within the practice and supporting high standards of care

- Are a qualified Advanced Clinical Practitioner with current NMC / HCPC / GPhC registration
- Hold an MSc in Advanced Clinical Practice and are a certified Independent Prescriber
Experience

- Primary care experience, including minor illness management
- Experience managing whole episodes of care while working collaboratively within a wider multidisciplinary team
Knowledge, Skills and Abilities
