Rosedean House Surgery is seeking an experienced Community Matron to join our primary care team.

Main duties of the job

The Community Matron will support patients of all ages with diverse and often complex health needs across the Liskeard community. They will provide personalised, holistic care for adults with long-term conditions, frailty, and multiple comorbidities helping patients live well at home, in care homes, or in other community settings. The Community Matron will play a central role in:

  • Enabling patients to remain independent and at home for as long as possible
  • Reducing avoidable hospital admissions
  • Coordinating care across primary, community, and social care services
This position is ideal for a clinician who is passionate about proactive, patient-centred care, enjoys working across a diverse community, and wants to make a meaningful difference in the lives of both older adults and younger adults living with complex health needs.

About us

Our founding vision by a committed father and daughter team remains central: delivering care with compassion, integrity and a personal touch, like the traditional family doctor. Over two decades later, that founding spirit still guides our work.
