Job responsibilities

As our Primary Care Transformation Manager, youll be at the heart of change leading key initiatives that modernise general practice, improve patient care, and align with NHS and OneMedicare priorities. Youll work closely with practice teams, Primary Care Networks (PCNs), Integrated Care Boards (ICBs), and other stakeholders to ensure seamless, high-quality service delivery that meets the needs of the local population. Your leadership will help embed efficient systems, support income generation, ensure compliance, and most importantly, make a meaningful impact on the lives of our patients.

What Youll Be Doing

  • Deliver transformation projects that improve access, efficiency, and patient outcomes.
  • Translate strategic goals into real-world improvements at practice level.
  • Drive innovation and implement modern general practice initiatives.
  • Support CQC readiness through audits and training.
  • Work with clinical and non-clinical staff to embed sustainable change and continuous improvement.
  • Monitor and improve performance on QoF, IIF, LES/DES, and other NHS income streams.
  • Champion excellent patient engagement and experience, growing the practice list size.
  • Build strong stakeholder relationships and promote OneMedicare locally.
  • Provide day-to-day leadership, coaching, and development for practice teams.

The building has served the local community since 1874 and is now home to a dedicated team, providing care to approximately 10,000 registered patients.
