Job summary
We are looking for Experienced Advanced Clinical Practitioners to join our multi-disciplinary teams at Leeds Walk-in Centre (Burmantofts, LS9) and LGI (Leeds General Infirmary). This is a new role due to the growth of the service and an enhanced model of care.
Main duties of the job
The Advanced Clinical Practitioner will assess, diagnose and plan care for patients; including prescribing of medications where clinically indicated. The ACP will manage acute presentations to the Walk-In Centre, as part of a team of autonomous Practitioners; in addition to using the clinical knowledge and skills to determine urgency and best pathway for patients presenting to the Walk-in Centre (streaming).Continuous professional development is key for our clinical workforce with the role having dedicated time for this and access to regular educational sessions. The full-time equivalent for us is 40 hours per week. Part-time can also be considered and we welcome conversations about flexible working.

- Supporting the wider MDT to ensure patients are seen by the right person
