Central London Community Healthcare NHS Trust and Healthcare Central London are working in partnership and offering 4 new exciting portfolio pharmacist roles! As a portfolio pharmacist, you have the opportunity to handle several roles within your portfolio. You will be working across the two organisations and gaining skills in both community health services and primary care settings.This will give you invaluable experience and opportunities to grow your pharmacy knowledge while offering you variety and a balance of work that suits you with indemnity insurance support.

Main duties of the job

Key components of the post when in the Community Health services element will include working as a medicines optimisation pharmacist in various settings (patient's home; bedded units; clinics etc.) and ensuring an integrated approach to the management of medicines in high-risk patients. This will involve working as part of a multi-disciplinary team, and conducting clinical medication reviews. The pharmacist will also be involved in policy development, incident monitoring, and implementing relevant medicines-related national guidance.

Within the Primary Care services element, the post holder will be working in Westminster as part of the Healthcare Central London provision of Clinical Phamacists to General practice.

The pharmacist will work as part of a multi-disciplinary team in a patient-facing role. The role will involve taking responsibility for areas of chronic disease management within the practice and undertaking clinical medication reviews to proactively manage patients with complex polypharmacy, especially for older people, people in residential care homes and those with multiple co-morbidities.

The pharmacist will provide primary support to general practice staff with regards to prescription and medication queries.

We operate several NHS contracts on behalf of our PCNs including a Community Dermatology Service; Community Cardiology Service and a Secondary Care Referral Service and an out-of-hospitals provision which is sub-contracted to our 33 General Practices.

Trust Wide / Primary Care Medicines Management Service Provision

To support the Head of Pharmacy/Head of Medicines Management in the implementation of the Medicines Management Strategy and organizational priorities in line with national and local policy. Examples of work streams include policy development, incident monitoring, implementing relevant medicines-related national guidance across the organization(s)

To provide an independent source of specialist advice and support to both community health services and primary care teams on clinical effectiveness of medicines and cost-effective medicines management

To co-ordinate and participate in collecting and recording appropriate quantifiable data/activity and intervention data to monitor key performance indicators to identify areas for improvement and development

To support medicines management projects and workstreams within CLCH and HCL

To support the Head of Medicines Management/ Head of Pharmacy to manage strategies for clinical and financial risk management, clinical governance and controls assurance for medicines management in collaboration with other Directorates

To devise, undertake and evaluate audits as agreed with managers

Management/Leadership

To support the evaluation of the effectiveness of this new portfolio role working across two care settings.

To ensure that communication with other health care professionals around medication related issues are dispatched in accordance with local policy.

To review systems of work, developing programs to improve efficiency where required, and monitor these changes
