Job summary
Yorkshire Health Network are looking for a caring and reliable Health Care Assistant to carry out Health Checks in the community for a short term project.
The post holder will be offered the opportunity to work some additional hours in our Enhanced Access Service, on evenings and weekends on a long term basis.
Main duties of the job
The Health Care Assistant will primarily be delivering health checks in the community. They will be a key member of the multidisciplinary team and will support the clinical team in delivering high quality primary care services to patients. Working under the direct supervision of the clinical team, the roles involves direct patient contact and is vital in promoting patients health and well-being.

Yorkshire Health Network is a federation of 16 GP practices in the Harrogate and surrounding area. It is governed by a Board of Directors, which brings together local practice representation and industry expertise to improve local healthcare.
On behalf of the 16 GP practices, Yorkshire Health Network provides the Enhanced Access service at three locations, Ripon, Knaresborough and Harrogate for all patients registered with a GP in the Harrogate and Rural District Area.
Yorkshire Health Network work in partnership with 4 Primary Care Networks (PCN's) providing proactive, personalised and integrated health and social care.
Yorkshire Health Network offer a private vaccination services for people registered with a GP surgery within the Harrogate and Rural District.

Job responsibilities
- Provide fundamental quality care for patients.
- Contribute to quality improvement.
- Ensure care environment is always clean and tidy meeting IPC requirements including the Sterilisation and cleansing of equipment.
- Support long term condition management including the community NHS Health Checks. Clinical tasks including but not limited to phlebotomy/ point of care testing, blood pressure monitoring, provide information for patients regarding weight and diet, checking emergency drugs, replacing expired equipment and height, weight and BMI measurements.
- Assist with the delivery of Quality & Outcomes Framework.
- Process and management of laboratory samples within point of care testing.
- Update equipment stock lists and report on stock levels to ensure timely ordering or stock and incorporating stock rotation.
- Prepare and maintain environments and equipment before, during and after patient care interventions including the clearing and re-stocking of consulting rooms.
- Assist in the assessment and surveillance of patients health and well-being.
- Undertake specific clinical activities for named patients that have been delegated and taught specifically in relation to that individual.
- Help to raise awareness of health and well-being and how it can be promoted.
- Assist with the collection and collation of data on needs related to health and well-being.
- Assist in vaccination and immunization clinics.
- Maintain accurate patient record using the clinical system.
- Adhere to infection control policies and safety regulations.
- Provide chaperone support for patients during clinical examinations if required.
