Job Summary: The Primary Care Advanced Nurse Practitioner (ANP) plays a crucial role in delivering comprehensive and patient-centered primary healthcare services. Working collaboratively with GPs, nurses, and other healthcare professionals, the ANP assesses, diagnoses, treats, and manages a wide range of acute and chronic medical conditions.
Responsibilities:
- Patient Assessment and Diagnosis:
- Conduct thorough health assessments, including medical history, physical examinations, and diagnostic tests.
- Formulate differential diagnoses and develop treatment plans in collaboration with the healthcare team.
- Treatment and Management:
- Prescribe medications, order and interpret diagnostic tests, and develop treatment regimens.
- Manage chronic conditions, promote health maintenance, and provide preventive care.
- Clinical Procedures:
- Perform minor procedures, such as wound care and joint injections.
- Provide contraceptive advice/treatments and other necessary medical interventions.
- Potential for coil fittings
- Patient Education:
- Educate patients and their families about health conditions, treatment options, and preventive measures.
- Encourage and support lifestyle modifications for improved health outcomes.
- Collaboration and Communication:
- Collaborate with GPs and other healthcare professionals to ensure coordinated and holistic patient care.
- Communicate effectively with patients, families, and colleagues to promote a positive healthcare experience.
- Documentation:
- Maintain accurate and up-to-date patient records, including progress notes, treatment plans, and other relevant documentation.
- Quality Improvement and Compliance:
- Participate in quality improvement initiatives and adhere to healthcare regulations and standards.
- Stay current with evidence-based practices and contribute to the development of clinical protocols.
Qualifications:
- Master's or Doctoral degree in Nursing clinical practice.
- Current and unrestricted Advanced Practice Registered Nurse (ANPR) license with NMC with previous experience of working in a general practice/primary care setting
- Certification as a competent Nurse Practitioner in the primary care specialty.
- Independent prescriber.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a team-based healthcare environment.
- Commitment to ongoing professional development and continuous learning.
Job Overview:
The Advanced Nurse Practitioner is a specialised healthcare professional responsible for providing expert care and treatment of conditions in a primary care setting.
TLC holds annual contracts to provide a number of services to our Federated general practices from the Eastleigh, Chandlers Ford and Romsey & North Baddesley areas. Services are currently provided at the following premises:
Abbeywell Surgery, Nightingale site, Romsey,
St Andrews Surgery, Eastleigh and
Knightwood Surgery premises, Chandlers Ford.
Contracts held:
To provide enhanced access appointments to general practices as follows:
Monday to Friday: 6.30pm 10.30pm
Saturdays: 9am 5pm
Sundays & Bank Holidays: Out of Hours appointments during a 4-hour session (shared with GPs in the team)
Currently appointments are provided in Romsey & Eastleigh
Transformation team:
Providing a holistic assessment for frail and/or elderly patients to prevent hospital admissions.
Complex Wound Care:
Provided to named general practices from Eastleigh currently provided at Knightwood Surgery premises, Chandlers Ford
Possible contract developments:
To provide a womens health service
To provide an enhanced contraceptive service to include coil fittings
To provide a spirometry service
To provide dermoscopy
To provide a vasectomy service
