Haddenham Medical Centre- Location: Haddenham
Unity Health- Locations: Chinnor, Princes Risborough, Long Crendon, Thame and Brill
Cross Keys Surgery- Locations: Princes Risborough and Chinnor
Base location:This role has no fixed base, the post holder will be working across all PCN practices
Salary:Dependent on experience
Salary will be commensurate with experience and aligned with the guidance set out in the NHS England Network Contract DES Contract Specification 2026/27

Main duties of the job
Phoenix Health PCN is seeking an enthusiastic Paramedic to join our PCN as part of the Additional Roles Reimbursement Scheme (ARRS). This is an exciting opportunity to be part of a team focused on delivering patient-centred care in a collaborative and supportive environment.
The post holder will work across our PCN network which covers 3 practices (Haddenham Medical Centre, Cross Keys Surgery and Unity Health). We have surgeries based in Princes Risborough, Chinnor, Haddenham, Thame, Brill and Long Crendon. The role will be the centre of our Housebound Visiting Service primarily, visiting patients at home to support them with their acute and chronic conditions.
Salary will be commensurate with experience, and in line with the guidance set out in the NHS England Network Contract DES Contract Specification 2026/27.
This role is a registered paramedic educated to degree level who acts within their professional boundaries.
Paramedics work independently within their scope of practice in the community, using their enhanced clinical assessment and treatment skills to assess and manage patients presenting with acute presentations which include minor illness or injury, abdominal pains, chest pains and headaches.

About the PCN
Phoenix Health PCN brings together three GP practices across Buckinghamshire, working collaboratively to deliver more coordinated, accessible, and proactive healthcare for our local communities. Our multidisciplinary team includes Clinical Pharmacists who support safe and effective medicines management, Pharmacy Technicians who assist with repeat prescribing and medication reviews, Care Coordinators who help patients navigate health and social care services, Social Prescribers who support patients with their health and wellbeing and a Paramedic who provides timely assessment and urgent care within the community. As demand grows and the needs of our population become increasingly complex, we are committed to strengthening our workforce further. We aim to have a team with dedicated professionals who can help us provide tailored support to our most vulnerable patients such as those who may struggle with long-term conditions, frailty, social isolation, or barriers to accessing care. Our goal is to enhance continuity of care, improve patient outcomes, and ensure that everyone receives the right care at the right time.
Duties and Responsibilities
A Paramedic is to undertake the following key responsibilities in delivering health services:
- Mainly, visiting patients at home and making a clinical assessment of their acute and/or chronic medical problems. Actioning further management accordingly such as arranging bloods or other investigations, referrals, arranging prescriptions, or admission if necessary
- Make decisions in relation to presenting medical problems, whether self-referred or referred from other healthcare workers within the organisation
- Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the organisations social prescribing service and, where appropriate, other community or voluntary services
- The role will provide a specialist Paramedic resource for health care professionals and service users, working in collaboration with other members of the Multidisciplinary Team.
- To work within the surgery, as an autonomous, accountable Paramedic, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment to deliver quality patient services.
Be able to:
- Perform specialist health checks and reviews within their scope of practice and in line with local and national guidance
- Perform and interpret ECGs
- Perform investigatory procedures as required and undertake the collection of pathological specimens including intravenous blood samples, swabs and other samples within their scope of practice and in line of local and national guidance
- Support the delivery of anticipatory care plans and lead certain services (e.g., monitoring blood pressure and diabetes risk of elderly patients)
- Provide an alternative model to urgent and same day GP home visit for the practice
- Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered
- Communicate proactively and effectively with all colleagues across the MDT, attending and contributing to meetings as required
- Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient healthcare and registration with the organisation
- Communicate effectively with patients and, where appropriate, family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings and treatment choices
