Are you an experienced clinical pharmacist with a passion for leadership and innovation? We are seeking a dynamic Advanced Practitioner Pharmacist Lead to shape and develop an integrated pharmacy team within our Primary Care Network (PCN). Within this position you will directly improve patient care and support the efficient running of a new model of general practice.
Main duties of the job
In this strategic and clinical leadership role, you will:
- Lead and grow a high-performing pharmacy team, embedding pharmacists and pharmacy technicians across our network practices.
- Drive the integration of pharmacy services to improve medicines optimisation, patient safety, and population health outcomes.
- Provide expert clinical input, mentorship, and strategic direction to ensure our pharmacy team delivers exceptional, patient-centred care.
- Collaborate with GPs, nurses, and the wider multi-disciplinary team to develop innovative, system-wide approaches to prescribing and long-term condition management.

Job responsibilities
To improve patients health outcomes and the efficiency of the Primary Care Network by providing direct, accessible and timely Clinical Medicines expertise, by working as part of a Multi-Disciplinary Team. To pro-actively transfer workload relating to medicines optimisation issues from other clinical staff to improve patient care, safety, thus freeing up clinical staff to spend more time on other clinical care:
- To provide relevant support to the primary care team.
- To promote safe, evidence based, cost effective prescribing in line with ICB requirements
- To provide expert pharmaceutical advice to Practices and the Medicines Optimisation Team
- To oversee PCN prescribing support to the designated Practices and team members in areas such as
o Hospital discharge letters
o Face to Face Medication reviews and follow up
o Implementation of prescribing changes
o Review of repeat prescribing
o Audit
