An exciting opportunity has arisen for a Trainee Health Coach to join our developing multidisciplinary team. Aylesbury Central Primary Care Network (AC PCN) is made up of 2 GP practices based in the central Aylesbury with a patient population of 40,000+ patients.

AC PCN Health & Wellbeing Coaches pride themselves on being a distinctive Health Coaching team, offering unparalleled services tailored specifically to our community's needs.

Our exemplary Health and Wellbeing Coaching service is recognised within the ICB for delivering exceptional care to patients with complex needs. Through collaboration with Health Coaches and the wider PCN teams, these patients are achieving remarkable outcomes, such as reversing diabetes, reducing medication requirements, and improving health outcomes that were previously unattainable using other interventions.

Our ambition is to expand the service to include children and family support, in addition to offering a formal Tier 3 weight management, including prescriptions of GLP-1s.

Main duties of the job

Role Summary

Health Coaches support people to take proactive steps to improve the way they manage their physical and mental health conditions, based on what matters to them. They support patients through behaviour change by developing their knowledge, skills and confidence in managing their health and care, to improve their health outcomes and quality of life. Behaviour change happens through coaching and motivating patients through multiple sessions to identify their needs and to set goals for themselves, and through providing access to interventions such as self-management education and peer support.

Health Coaches manage and prioritise a caseload, according to the needs, priorities and support required by individuals. They identify when a persons needs are beyond the scope of the Health Coach role and refer them back to other health professionals or organisations. Health Coaches may work with people by phone, by video consultation, face-to-face, on a one to one basis and in groups.

Health and wellbeing coaches will have access to ongoing supervision, skills development, and support to continue building their skill set and experience within the role.

Primary duties and areas of responsibilities

Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement their personalised health and care plan.

Provide personalised support to individuals, their families, and carers to ensure that they are active participants in their own healthcare; empowering them to take more control in managing their own health and wellbeing, to live independently and improve their health outcomes through:

1)Providing 1-1 and group Health Coaching support for patients based on what is most important to them, with the aim of:

-Improving patients knowledge, skills, and confidence in managing their health condition/s.

-Empowering and motivating patients to set goals to improve their health and outcomes.

-Supporting patients to change their health related behaviours.

-Supporting with problem solving to aid patients' ability to adapt and sustain their lifestyle changes.

2)Learning skills to adapt the approach per patient to keep it personalized, aiding development of own coaching style.

3)Managing own caseload, in accordance with the needs, priorities and support required by individuals. Along with a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, because the persons needs are beyond the scope of the Health Coaching role e.g. when there is a change in the persons health condition or where un met clinical support needs are identified.

4) Working within a wider Primary Care team, such as the Care Co-ordinators and Social Prescribers to connect patients to community-based activities/additional social support.

5) Working with the Long-Term Conditions (LTC) team to deliver specific health advice, where appropriate.

6) Provide support to local community groups and work with other health, social care, and voluntary sector providers to support the patients, health and well-being holistically.

7) Help to establish and co-ordinate patient groups to aid self-management, peer support, and the creation of patient communities.

8) Ensure that GPs, practice nurses, practice pharmacists and other members of the primary care team understand the Health Coaching role, how to refer patients, and which patients may particularly benefit from Health Coaching.

9)Attend and contribute to team, practice, and PCN meetings and events as required by the service.

10)Work flexibly, engaging in appropriate projects and adapting to the needs of the service and client group while maintaining the integrity of the role.

11) Raise awareness within the PCN of shared decision making and decision support tools and supporting patients in shared decision-making conversations.

  • NVQ Level 3 Health Trainer qualification or other relevant professional academic qualification
