Main duties of the job

An exciting opportunity has arisen for a Senior Health and Wellbeing Coach to join our developing multidisciplinary team at Arc Bucks Primary Care Network.

We are looking for a Senior Health & Wellbeing Coach to support our practices by utilising your health coaching skills to support individuals with lower levels of patient activation. You will help them to develop their knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers. You will also be responsible for line managing and providing supervision to other Health & Wellbeing Coaches.

You will lead the Health & Wellbeing team and will support several practices, tailoring your work to the needs of their populations and to your own interests/skills. Upon recruitment, you will be offered training to become a Level 1 Health Coach if this has not already been completed, alongside various other relevant training opportunities.

Primary Duties and Areas of Responsibility

Line Management

  • Conduct one to one supervision meetings with direct reports to identify support needs, safeguard their personal wellbeing and encourage learning and development.
  • Monitor the performance of direct reports, ensuring that all duties are completed to a high standard and that patients are receiving quality support.
  • Identify and mitigate any risks within the services and ensure all policies and procedures are followed by team members.
  • Enable team members to achieve success by providing motivation, encouragement and expertise to support development.
  • Plan and lead team meetings.
  • Attend monthly PCN Senior Management meetings

Person-centred care and support

  • Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement their personalised health and care plan.
  • Support patients to establish and attain goals set by the person, based on what is important to them
  • Provide ongoing motivational support and follow up to ensure long term behaviour changes.
  • Work with the long-term conditions (LTC) teams to deliver specific lifestyle advice (pre-diabetes, hypercholesterolaemia, hypertension)
  • Work to decrease the prevalence of treatable LTCs by undertaking health checks if required
  • Deliver a range of group education programmes dependent on patient needs
