Main duties of the job

Phoenix Health Primary Care Network (PCN) is looking to recruit 3 Advanced Nurse Practitioners, to work within our innovative and friendly multidisciplinary healthcare team. Phoenix Health PCN provides a range of services to the patients, communities and practices within the PCN area which is based at our offices in Chinnor and covers the practice area of our 3 member practices: Cross Keys Practice, Haddenham Medical Centre and Unity Health.

The 3 Advanced Nurse Practitioners will be part of an exciting, innovative service development, working as part of a multi-disciplinary team in our care homes and providing minor illness clinics, working as part of a general practice team.

The post holder will be an experienced nurse who, acting within their professional boundaries, will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within general practice, in care homes and for the frail elderly living in the community.

  • Advanced Life Support provider
  • Independent non medical prescribing qualification
