Join our Practice Team as a Pharmacy Technician!
Presteigne Medical Centre is looking for a dedicated Pharmacy
Technician to help us proactively help patients stay safe and well out of
hospital, as well as reducing the demands on General Practice. Working together
with the Practice Team, this position plays a crucial role in ensuring we support
patients to get the best outcomes from their medicines. You will work with the surgery promoting high
quality and cost-effective prescribing practice.
The role will include development of practice based prescribing initiatives to support improved coordination and effective pharmacy administration within general practice, especially with relation to repeat prescribing systems and processes.

Presteigne
Medical Centre is a small, rural Medical Centre that sits on the Powys
border. We are recruiting a Pharmacy
Technician to assist the Practice in providing patient-centred care by
reviewing optimising medication and monitoring medicines. We want to be able to
support patients more efficiently to make best use of medicines, ensuring safe
and effective repeat prescribing. Our hours of Service are 08:00 to 18:30.

more time for GPs to support complex patients.

with transfer of care and its impact on patients and their medication.

family in July 2023. Shropdoc holds the contract to provide general medical
services (GMS) to the patients of Presteigne and surrounding areas. We are a
small, dedicated practice team serving a population of 3600 patients.
Presteigne sits on the border of Powys in a rural location.

Job responsibilities
KEY TASKS AND RESPONSIBILITIES
- To undertake medicines reconciliation in
accordance with practice protocols and update patients medical records/
prescribing accordingly.
- To respond to discharge outpatient
letters and update records and test requests accordingly, and effectively
communicate relevant changes to prescribers, dispensers and patients.
- To support the achievement of the
practices prescribing and QAIF targets.
- To work with the Clinicians to
contribute the review and development of repeat prescribing protocols /
Standard Operating Procedures to improve the efficiency and effectiveness of
repeat prescribingsystems.
- To provide advice to GPs, staff,
patients in changes to prescribing to support the improvement of prescribing
safety, quality, and cost effectiveness.
- To participate in medication/ treatment
audits.
- To ensure appropriate drug monitoring is
undertaken by requesting appropriate blood tests/examinations as per protocol.
- To oversee any LESs/DESs are adhered
to with regards to prescribing/monitoring and audit
- To prepare evidence-based resources and
information to support the implementation of rational cost-effective
prescribing. (Green initiative/ All Wales Prescribing Guidance)
- To support the practice based clinical
staff to provide medication reviews, patient information leaflets and medicines
awareness; and undertake community visits as appropriate.
- To take a proactive approach to the safe
handling of prescriptions, including assessment of risk and making
recommendations for improvement.
- To respond to medication queries from
patients and staff in a professional manner, referring to the appropriate GP in
accordance with practice protocols. This will include general queries, complex
medication requests, arranging appropriate alternatives when medicines are
unavailable and synchronization of repeat medication.

to monitor INR annual review
