We are currently recruiting additional people to join our very friendly team of healthcare professionals in our Care Co-ordination Centre at Longbow Close, Shrewsbury.

The role of the Urgent care practitioner will be to work in three blended specialities, Care Co-ordination (CCC) Single point of access (SPA) the Oncology helpline and Telephone triage.

Main duties of the job

The Role Urgent Care Practitioner

Youll be joining the multi-disciplinary team of healthcare professionals at our Care Coordination Centre in Shrewsbury. As an Urgent Care Practitioner your focus will be on providing a telephone-based service to patients and healthcare professionals to coordinate the best pathways for patients, from calls that have been escalated from our Call Handlers.

Your duties will include, although not be limited to:

  • Liaising with healthcare professionals to find suitable admission options for patients, and coordinating and arranging admissions
  • Delivering a telephone helpline service for oncology patients
  • Providing telephone triage for out-of-hours urgent care
  • Assisting with admission avoidance and therapeutic interventions
  • Working closely with our partners in community care services
