KEY TASKS AND
RESPONSIBILITIES
DUTIES & RESPONSBILITIES
The post holder will:-
Provide expert generalist care
within the Primary Care Setting, namely Public Health Nursing and First Contact
Nursing across the Community.
Provide specialist care for
chronic disease management working autonomously in primary care premises and
patients homes to coordinate the care needs of patients.
Support the Nurse Manager in
developing Primary Care Nursing services across the Community.
Delegate duties to junior staff,
as appropriate e.g. junior registered nurses, Health Care Assistants, student
nurses.
On occasions be required to work
within other areas of the Community Resource Team.
Be expected to support junior
staff within their sphere of responsibility, including junior registered
nurses, student nurses and Health Care Assistants and contribute to the
Continuing Professional Development of Practice Nursing and other Primary Care staff.
Deliver sensitive information
during clinical consultations in General Practice. Discuss and facilitate the
patients decision-making process regarding future treatment plans.
Utilise motivational and
negotiating techniques to encourage patients to follow the best course of
action or treatment when they are resistant to change, frightened or in denial.
Utilise counselling skills such as
empathy, listening, unconditional positive regard and congruence when informed
consent is required, particularly from vulnerable adults and those with special
needs and learning difficulties.
Demonstrate specialist knowledge
(though relevant diplomas and postgraduate studies) during clinical
consultations.
Participate in the development of
clinical guidelines using evidence based patient centred care.
Communication:-
Work in partnership, ensuring good
communication with the multidisciplinary team e.g. medical staff, community
nurses, Social Services, Physiotherapy, Occupational Therapy and other areas
appropriate.
Provide information and guidance
pertinent to the plan of care to clients and their next of kin. This will
involve providing and receiving complex, sensitive or contentious information
where persuasive, motivational, negotiating, training, empathic or reassurance
skills are required.
Provide information and guidance
to Practice staff.
Ability to work as part of a team
and have good communication skills.
Be expected to participate in team
meetings.
Analytical & Judgemental Skills:
The post holder will be responsible for:
Acting on then informing and
advising the Practice Manager / Nurse Manager of actual or potential problems
with regard to patients, staff, etc, and as necessary ensure appropriate action
in resolving such problems is taken.
Be required to make decisions on
care provided, making referrals as appropriate also supporting junior staff in
this role.
These may involve judgements
involving complex facts or situations, which require analysis, interpretation
and comparison of a range of options, for example blood results.
Investigate incidents and
complaints, completing an action plan from lessons learned, ensuring all staff
are involved in and informed of any changes to policies and procedures.
Planning & Organisational Skills:
Be responsible for organising
their own workload supervising the workload of junior registered nurses, Health
Care Assistants, and student nurses. This will include assisting in the
supporting/teaching of newly appointed registered nurses, student nurses and
Health Care Assistants.
Contribute in development and
implementation of primary care objectives, which reflect past, and future
Health Board objectives.
Physical Skills:
The post requires highly developed
physical skills, where accuracy is important, but there is no specific
requirement for speed.
Undertake venepuncture procedures
following appropriate training and on completion of care competencies.
Undertake ECG procedure following
a period of training, reporting abnormalities to the medical team.
Able to maintain (on a frequent
basis) periods of prolonged concentration during clinical assessments and
examinations of patients.
Patient / Client Care:
The
post holder will be responsible for the development of care packages,
including:-
The assessment, planning,
delivery, evaluation and updating of individual plans of care, acknowledging
personal beliefs, identity and preferences of parents/carers.
Undertake direct nursing care for
patients across a range of procedures, who may require wound care e.g. removal
of sutures.
Enable patients to achieve
physical comfort assisting in minimising client discomfort and pain.
Participate in basic health
education of patients and carers.
Contribute to the ongoing support
of patients and others significant to them, maintaining the patients
interests, their identity and their wellbeing.
Policy & Service Development:
Ensure that Practice and Statutory
Policies and Procedures are followed and implemented, and report any problems
including proposing changes to working practices or procedures.
Financial & Physical Resources:
At all times, assist the Practice
Manager in the management of resources within their sphere of responsibility.
Have an awareness of budget and
spending trends within the Practice, and associated skill mix/staff resources.
Where appropriate, will monitor
and record expenditure e.g. medical and surgical, staff costs etc.
Human Resources:
Be responsible for day to day
supervision of staff, including clinical supervision and training which may
include basic HR advice.
Be expected to assist in the
identification and achievement of staff training and development needs through
the implementation of Health Board policies and procedures.
Ensure a learning environment to
encourage staff development.
Advice and arbitration where there
may be differences of opinion amongst staff.
Will act as a mentor for student
nurses, return to practice nurses.
Have the opportunity to develop,
through IPR, their own clinical and managerial skills to the benefit of the
Practice.
Ensure that staff working within
the area are aware of their responsibilities within Practice policies.
