Patient Coordinator
Our Patient Coordinators are the heart of our organisation,
connecting our patients with the service they need and looking after their
journey from referral to discharge. They are friendly, knowledgeable and
empathetic, with great attention to detail plus their work makes a difference
every day.
We're an independent provider of health services, based in
Bristol and providing services across the South West. Our aim is to provide
care closer to home in a timely way.
As well as our NHS clinics, we also provide a self-paying
Minor Surgery service, and this is where we are looking to appoint another
Patient Coordinator. Although the focus of the role will be enabling patients
to access our Minor Surgery service by matching their needs to an appropriate
surgeon, you will also provide cross cover support to our NHS services.

You'll be based at our headquarters in Aztec West, alongside
the other Patient Coordinators. We currently work on a hybrid basis with the
team coming into our office for approximately 50% of their working hours.
Working hours: up to 30 hours per week over 4 days, must
include a Friday.

Main duties of the job
You will work in the centralised clinical administration services based
out of Aztec West where the ability to use your own initiative, work in an
empowered manner, multitask and prioritise activity are all key. In your role as Patient Coordinator you will
be the initial point of contact, mostly by phone, for patients referred by
their GP practices into GP Cares services.
You will be responsible for receiving patient referrals and enquiries
online, via email or by telephone and managing the patient documentation
through triage to the clinic. You will
manage clinic schedules, booking patients into the appropriate clinics for
diagnostic tests, assessment and treatment while ensuring that clinic time is
managed efficiently. Excellent attention to detail is required and maintaining
confidentiality in the management of critical patient information.

Salary and annual leave are pro rated for part time working
