Job summary

We are looking for a motivated Practice Nurse to join our Enhanced Access Service for Havant and Waterlooville PCN. Youll be part of a supportive team of GPs, nurses, HCAs, and operational staff, delivering high-quality care to patients referred by their GP practice for evening and weekend appointments.

This is an exciting opportunity to use your skills in a flexible role, with the chance to work across two local sites while making a real difference to the community.

Main duties of the job

  • Running treatment room clinics: dressings, cervical screening, immunisations & family planning.
  • Supporting long-term condition management (Asthma, COPD, Diabetes, Hypertension, CVD).
  • Promoting healthy lifestyles through screening and patient education.
  • Maintaining safe clinical practice, including safeguarding and infection control.
  • Working collaboratively within a multi-disciplinary team.

Level 1: Dressings

Level 2: Family planning to include contraceptive initiation and review, and immunisations.

Level 3: Chronic disease management (Asthma, COPD, Diabetes, Hypertension, CVD)

The post holder will:

Provide treatment room services, and others, depending on competency level and scope of practice. To include, but not limited to; wound care, cervical cytology, swabs, family planning such as contraceptive initiation and checks, Chronic Disease Management (Asthma, COPD, Diabetes)

Screening; Undertake opportunistic screening including BP, height, weight, BMI, urinalysis, smoking status and alcohol intake.

Health promotion: offer education and motivation for patients to change unhealthy behaviours. Recognise a patients readiness to change. Develop and maintain health promotion material.

Infection control: Ensure that the highest infection control standards are met.

Emergency management: Be competent in anaphylaxis and resuscitation techniques.

Safeguarding: Be aware of recognising the need for safeguarding children and adults, raising concerns appropriately.

Administration and paperwork: Arising directly from this caseload (record-keeping, other written communications with local GP surgeries as required) and ensuring all appropriate templates and coding is completed for QOF, LCS, DES.

Team meetings: Essential to the delivery of team-based care, contributing to discussion of clinical practice standards, developing protocols, mutual professional support for colleagues, you will need to attend meetings and actively participate in these.

Personal CPD (continuing professional development): A mix of in-house events, private study and external education events are expected, to remain up to date as is required by your professional registration.

Company-wide training. Join and engage in events to maintain a good understanding of the businesses needs and colleagues.

Adherence to KPIs without jeopardising the safety of the patient.

Identify opportunities and to proactively encourage health promotion and education to all patients.

To be professionally and legally responsible and accountable for all aspects of clinical decision making within your clinical competence.

To provide mentorship and support to junior staff as and when required.

To assist in the development of new models for service delivery which have a positive impact on the health economy.

Undertake effective multidisciplinary liaison across the primary and secondary care interface and PHL Clinical and Operational Team.

Actively participate in investigation of incidents, complaints and investigations as required.

Engage with education and development, and take a responsibility to meet learning objectives as set out in your appraisal and maintain levels of CPD in line with your professional registration.

Integrate within the clinical and operational teams to identify and deliver service requirements and improvements.

Support junior members of the team, offer educational and clinical supervision opportunities and guidance through clinical discussion.
