Blossom Health is
looking to recruit a Prescribing ANP/ Prescribing Paramedic for up to 10
sessions working across our Havant & Bedhampton sites Monday to Friday.

We are a very friendly practice
with a vision for innovative development working together across two sites. The
practice has exceptional staff retention among their existing clinical and
non-clinical team. We have a strong Multi-Disciplinary Team via the PCN
including Home Visiting Team, First Contact Physios and Health & Wellbeing
coaches.
Main duties of the job
As an Advanced Clinical Practitioner
(ACP) at Blossom Health working as part of a team, your role is so very
important in providing an efficient primary care service to our patients with a
caring and respectful disposition. Working independently but with a supportive
team, through face-to-face consultations in a planned care and urgent care
clinic or home visits, or via remote telephone triage you will be accountable
for delivering safe, clinical decision-making and expert care to a high
standard. Acting within your professional boundaries, your duties will include
aspects of the patient journey including initial history taking; clinical
physical assessment; diagnosis; treatment; prescribing medication and
evaluation of their care.
You will be required to work completely
independently in an autonomous role at an advanced level of practice. Advanced
practice will include evidence of working within the four pillars, clinical,
education, research and leadership.

- Work autonomously and be accountable for their own professional actions.
- Assess undifferentiated patients (with no restrictions on age ranges from babies to adults) with a range of acute, non-acute and chronic medical conditions.
- Take history, make appropriate physical examinations, formulate differential diagnoses and formulate/manage treatment plans when working in any clinical environments.
- Refer and facilitate patient pathways through appropriate referrals to primary, community, secondary and tertiary care for patients when necessary, such as 2-week wait, urgent and routine referrals to specialities.
- Request
investigations and be able to interpret pathology and radiology results to
ensure appropriate treatment and/or onward referrals.
- Prescribe
medication as required within your clinical competence.
- Recognise
the need and initiate emergency treatment to save life or to safeguard the
interests of the patient and refer to emergency services when appropriate.

any social needs, safeguarding concerns and act appropriately to safeguard the
patient.

- Registered with the NMC/HCPC. Non-medical prescriber. Completion of MSc Advanced Practice Pathway or equivalent. Minor illness and minor injury qualification. Teaching and mentorship qualification. Level 3 safeguarding of adults and children. Must be able to demonstrate and meet competencies of all mandatory training as required within the policies of the organisation. Must be authorised to work in the EEC. Must be able to clear and enhanced DBS check. IRMER qualification (service specific). Red dot qualification (service specific). Experience in lab result interpretation.
