We are looking to recruit for a Advanced Nurse Practitioner, (ANP) to join our team in the New Forest to lead the delivery of Enhanced
Access appointment provision for the local Primary Care Networks (PCNs). Based
at the Ringwood Medical Centre.

join the team based in Ringwood Medical Centre.
Hours: Monday OR
Thursday 1830-2130

Main duties of the job
The post holder will work as part of a team for Primary Care Extended Access Service based in Lymington, Ringwood and Winchester.
The post holder will provide clinical assessment and treatment for the delivery of a safe, efficient and reliable clinical service. They will work closely with the Clinical Lead, and Service Manager. They will form links and develop relationships with secondary care and GP Service colleagues and those working in the wider healthcare community. The post holder will ensure patients receive optimal care in the correct setting within the healthcare system. Health policies and standard operating procedures will be adhered to and, if relevant, refined
In the planned primary care clinic the patient will undergo clinical assessment and review, examination, appropriate investigations requested eg x-rays, scans, blood tests; and a diagnosis and management plan will be agreed. On occasions onward referrals may be needed to specialities and an expectation of a credible referral should be made.

The service is staffed by a team of GPs, Nurses, Paramedics, Healthcare assistants and non clinical workforce.
PHL now are working with the local healthcare community to provide the extended access Primary Care Service appointments in line with GP Improved Access hub criteria, with full access to patient medical records using EMIS. The purpose is to work collaboratively and support improving patient outcomes and experience for patients in Primary Care

- Work autonomously and be accountable for his/her own professional actions.
- Assess undifferentiated patients with a range of acute, non-acute, chronic medical conditions, minor illness and minor injury. Take a patient history, make appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans when working in a clinic or home visit environment.
- Formulate a management plan for the patient. And/or to use other sources of healthcare professionals (such as GP or Pharmacists) to if required to support diagnosis or next course of action
- Refer and facilitate patient pathways through appropriate referrals to primary, community, secondary and tertiary care

- Qualified non medical prescriber.
- Registered with NMC / HCPC
- Experience working in a primary care planned care clinics and same day/urgent care clinics
- Experience of Chronic Disease Management
- At least three years post qualified
- At or working towards Masters qualification / or equivalent experience demonstrating advanced clinical practice
- You must be competent at taking a clinical history excluding red flags and undertake a physical examination of the patient for patients presenting with undifferentiated conditions
- You will be expected to ensure that mandatory education is kept updated as required by the policies of the organisation
