Job summary

PHL Group has an exciting opportunity for an experienced General Practitioner to join the Special Allocations Scheme (SAS) team, which is a specialised GP service. The service provides primary care to patients across Hampshire and the Isle of Wight who have been removed from their mainstream surgeries.

Patients can access the service via a GP telephone consultation clinic which is held twice a week, or a face-to-face clinic which is held once a week in one of the four sites we have across Hampshire, on a rotational basis. Along with the SAS team, these sites provide a safe environment for both clinicians and patients during these face-to-face appointments.

Outside of these clinics we offer emergency appointments every day where a patient can contact a GP for urgent advice. The service operates on System One and appointments are 15 minute slots, with time for admin. The Special Allocation Scheme currently has less than 100 patients registered, which allows the team to provide high quality and dedicated care.

This is a fantastic opportunity to provide GP services to patients who may be considered vulnerable.

Hours

This role would suit someone who is looking for a permanent, part-time role.

Mondays and Wednesdays on a 4 week rolling rota.

Main duties of the job

The post holder will primarily provide clinical assessments for our registered patients via both telephone and face-to-face consultations. They will work closely with the wider SAS team to develop and grow this unique service, building close links and relationships with secondary care and GP service colleagues, and those working in the wider healthcare community. The post holder will ensure that patients receive optimal care in the most appropriate setting, whilst delivering the objectives of the service.

Qualifications

Essential

  • Active registration with GMC. MRCGP. Level 3 safeguarding of adults and children's. Must be able to demonstrate and meet competencies of all mandatory training as required within the policies of the organisation. Must be authorised to work in the UK. Must be able to clear an enhanced DBS check.

Desirable

  • Experience of chronic disease management, experience of telephone triage assessment and relevant pathways/electronic prescribing.
