Purpose
The post holder will work as part of a team for Lymington GP-led Urgent
Treatment Centre. They will have access to appraisal, teaching and training
and will be encouraged to pursue professional development.
The post holder will provide clinical assessment and treatment
(assessing and diagnosing patients) for the delivery of a safe, efficient and
reliable clinical service. They will
work closely with the Clinical team, Clinical Lead, and Service Manager. They will form links and develop relationships with
secondary care and GP Service colleagues and those working in the wider
healthcare community. The post holder will ensure
patients receive optimal care in the correct setting within the healthcare
system. Health policies and standard
operating procedures will be adhered to and, if relevant, refined
In the clinic the patient will undergo
clinical review, urgent investigations, eg x-rays, scans, blood tests; and a
diagnosis and management plan will be agreed.
Sometimes the patient is discussed with or seen by a specialist if the
problem is complex. If possible,
treatment will be started and the patient discharged home to be followed up
by the patients own GP.
The service is staffed by a team of
GPs, Nurses, Paramedics, Healthcare assistants, Plaster Technicians and non
clinical workforce.
Service Background
NHS England performed a review of Urgent and
Emergency care in 2013 and reported that patients consistently reported being confused
about where they should access urgent care, with a wide disparity in
offerings at services with a wide range of names including Minor Injury
Units, Primary Care Centres, Walk-in Centres and Urgent Care Centres.
PHL now are working with the local healthcare
community to implement Urgent Treatment Centres at Lymington as part of the
wider model of care with the purpose of simplifying patient pathways,
improving patient outcomes and experience and from a system perspective and
support a reduction in demand on ambulance and acute hospital services.

Key accountabilities
Clinical Duties
To
undertake assessment, diagnosis and management of adults and children
presenting with a wide range of minor injuries and illnesses. utilising
evidence based knowledge and skills as required.
To
develop clinical practice to be able to assess undifferentiated patients with
a range of acute, non-acute, chronic medical conditions, minor illness and
minor injury. Take a patient history, make appropriate physical examinations,
formulate differential diagnoses and carry through management treatment plans
when working in the UTC.
Formulate
a management plan for the patient.
And/or to use other sources of healthcare professionals (such as GP or
Pharmacists) to if required to support diagnosis or next course of action
Refer
and facilitate patient pathways through appropriate referrals to primary,
community, secondary and tertiary care
To
be clinically competent to administer medication using Patient group
Directions in line with department guidelines.
Recognise
the need and initiate emergency treatment in order to save life or to
safeguard the interests of the patient and refer to emergency services when
appropriate
Recognise
any social needs, safeguarding concerns and act appropriately to safeguard
the patient
To
ensure that patients are safely discharged from our care with appropriate
instructions for after care and follow up
Adherence
to KPIs without jeopardising the safety of the patient
Identify
opportunities and to proactively encourage health promotion and health
education to all patients
To
be professionally and legally responsible and accountable for all aspects of
clinical decision making within your clinical competence
To
take an active role within the cross-disciplinary, multi-skilled team to
ensure service quality in line with corporate goals and strategies and
provide mentorship and support to junior staff when required
Take a person-centred approach to
service delivery, based on clear understanding and experience of the modes
for service delivery.
To
assist in the development of new models for service delivery which have a
positive impact on the health economy
Maintain and develop clinical
competence and knowledge of care delivery within primary care and urgent care
setting using both formal and informal training methods.
To work within the professional Code
of Conduct and other directives as defined by the NMC/HCPC
To undertake appropriate personal and
professional development to meet the requirements of NMC/HCPC Registration.
To participate in regular performance
reviews with the Clinical Lead.
To
undertake further specialist training as dictated by service needs and the
changing requirements of service provisions

The
post holder will be expected to contribute to and support the development of
the service. It is expected that the
post holder will:
Work
to improve non-elective care pathways within the healthcare community.
Communicate
with the Governance Lead and support the delivery of a high quality service
and raise any governance issues that may arise.
Support
the development of junior team members. This may involve Student Nurses
sitting in and seeing patients where appropriate.
Work
with the Manager to ensure training,
significant events, complaints, audits, surveys and other professional
service development activities are achieved to ensure the smooth running of
the service.
Maintain
continued education through attendance at courses and/or study days as
necessary to ensure professional development requirements for on-going
professional registration and licensing are met.
To
take every opportunity to self develop and engage in a personal development
plan.
To
work as a reflective practitioner.
Support
the clinical lead in meeting their aims and objectives.
Skills and Experience
Essential
Qualified non medical prescriber.
Registered with NMC / HCPC
Experience working in a primary care and /or urgent
care setting
At least three years post qualified
At or working towards Masters qualification / or
equivalent experience demonstrating advanced clinical practice
You must be competent at taking a clinical history excluding
red flags and undertake a physical examination of the patient for patients
presenting with undifferentiated conditions
You
will be expected to ensure that mandatory education is kept updated as
required by the policies of the organisation
You
will be expected to maintain a clinical portfolio that evidences advanced
heath assessment skills and competencies.
Excellent
communication and interpersonal skills written, verbal (Particularly
listening skills)
Clear
understanding of professional responsibility and accountability.
Good
organisational skills and ability to manage and prioritise own workload
Good
clinical skills in assessment, diagnosis and management of acute Primary
care presentations of minor illness and injury
Be
able to clear an enhanced DBS check
Must
be authorised to work in the EEC
Desirable
A&E
experience
Minor illness qualification or similar
Interpretation of lab results training
IRMER trained
Teaching and/or Mentorship qualification
Experience of working in a multi-disciplinary team
Experience of Chronic Disease Management
Familiar with Internet Healthcare resources
