Job summary

  • Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions. Take history, make appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans when working in a clinic or home visit environment.
  • Through telephone clinical assessment, obtain a relevant and sequential history of the presenting complaint to determine a working diagnosis.
  • Formulate a management plan for the patient. Use other sources of healthcare professionals (such as GP or Pharmacists) if required to support diagnosis or next course of action.
  • Ensure competent to work under PGDs and supply medication under the direction of PGDs when assessed as appropriate.
  • Recognise the need and initiate emergency treatment in order to save life or to safeguard the interests of the patient and refer to emergency services when appropriate.

Main duties of the job

  • Registered with NMC / HCPC.
  • Experience working in a primary care and /or urgent care setting.
  • At least three years post qualified.
  • At or working towards degree level qualification / or equivalent experience.
  • You must be competent at taking a clinical history face to face and/or on the telephone, excluding red flags and undertaking an physical examination of the patient for patients presenting with undifferentiated conditions.
  • You will be expected to maintain a clinical portfolio that evidences advanced heath assessment skills and competencies.

Desirable

As an Urgent Care Practitioner you will be working as part of a team, providing an efficient integrated urgent care service to our patients with a caring and respectful disposition. Working independently via telephone and/or through face to face consultations and/or home visits, you will be accountable for delivering safe clinical decision-making and expert care to a high standard. Acting within your professional boundaries, your duties will include all (or part) of the patient journey including initial history taking; clinical assessment; diagnosis; treatment; supply of medication via PGDs and evaluation of their care. Urgent Care Practitioner (trainee) Urgent Treatment Centre

Reports to

Clinical Lead Urgent Treatment Centre

Purpose

The post holder will work as part of a team for Lymington GP-led Urgent Treatment Centre. They will have access to appraisal, teaching and training and will be encouraged to pursue professional development.

The post holder will provide clinical assessment and treatment (assessing and diagnosing patients) for the delivery of a safe, efficient and reliable clinical service. They will work closely with the Clinical team, Clinical Lead, and Service Manager. They will form links and develop relationships with secondary care and GP Service colleagues and those working in the wider healthcare community. The post holder will ensure patients receive optimal care in the correct setting within the healthcare system. Health policies and standard operating procedures will be adhered to and, if relevant, refined

In the clinic the patient will undergo clinical review, urgent investigations, eg x-rays, scans, blood tests; and a diagnosis and management plan will be agreed. Sometimes the patient is discussed with or seen by a specialist if the problem is complex. If possible, treatment will be started and the patient discharged home to be followed up by the patients own GP.

The service is staffed by a team of GPs, Nurses, Paramedics, Healthcare assistants, Plaster Technicians and non clinical workforce.

Service Background

NHS England performed a review of Urgent and Emergency care in 2013 and reported that patients consistently reported being confused about where they should access urgent care, with a wide disparity in offerings at services with a wide range of names including Minor Injury Units, Primary Care Centres, Walk-in Centres and Urgent Care Centres.

PHL now are working with the local healthcare community to implement Urgent Treatment Centres at Lymington as part of the wider model of care with the purpose of simplifying patient pathways, improving patient outcomes and experience and from a system perspective and support a reduction in demand on ambulance and acute hospital services.

Key accountabilities

Clinical Duties

To undertake assessment, diagnosis and management of adults and children presenting with a wide range of minor injuries and illnesses. utilising evidence based knowledge and skills as required.

To develop clinical practice to be able to assess undifferentiated patients with a range of acute, non-acute, chronic medical conditions, minor illness and minor injury. Take a patient history, make appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans when working in the UTC.

Formulate a management plan for the patient. And/or to use other sources of healthcare professionals (such as GP or Pharmacists) to if required to support diagnosis or next course of action

Refer and facilitate patient pathways through appropriate referrals to primary, community, secondary and tertiary care

To be clinically competent to administer medication using Patient group Directions in line with department guidelines.

Recognise the need and initiate emergency treatment in order to save life or to safeguard the interests of the patient and refer to emergency services when appropriate

Recognise any social needs, safeguarding concerns and act appropriately to safeguard the patient

To ensure that patients are safely discharged from our care with appropriate instructions for after care and follow up

Adherence to KPIs without jeopardising the safety of the patient

Identify opportunities and to proactively encourage health promotion and health education to all patients

To be professionally and legally responsible and accountable for all aspects of clinical decision making within your clinical competence

To take an active role within the cross-disciplinary, multi-skilled team to ensure service quality in line with corporate goals and strategies and provide mentorship and support to junior staff when required

Take a person-centred approach to service delivery, based on clear understanding and experience of the modes for service delivery.

To assist in the development of new models for service delivery which have a positive impact on the health economy

At least three years post qualified
