An exciting opportunity has arisen within Partnering Health Limited to
expand the Integrated Urgent Care Paediatric Desk.
PHL are an innovative healthcare provider knownfor providing
a range of the highest quality care using remote technology.
We are proud to offer a service that is paediatric specific where
our team provide clinicalcare remotely to children and young people within
Hampshireaged0-16 years.
Based in our Whiteley office, the service consists of a small team
of paediatric and adult trained nurses with a variety of skills and qualifications
which they use to clinically assess patients and support families by telephone
and video consultation. The service runs from 10am to 10pm 7 days
aweek, providing care for more than 1100 patients per month.
Our team are proud of the high standard of care provided which wasreflected
in the latest CQC inspection wherethe service was rated 'Good'.

Main duties of the job
Within the PHL Paediatric Desk, we are responsible for the
remote clinical assessment of children and their families who
contact 111 for support or advice. We take a full and comprehensive
history, including any factors relating to safeguarding concerns, which allows
us to provide the most appropriate outcome such as home management, face
to face appointments with OOH GP or Urgent Treatment Centre, or refer to
Emergency Departments and Ambulance Services for more urgent situations.
As part of the team, you will receive regular structured
appraisals and 1:1 sessions,as well as informal and formal
training supporting education and learningin practice. You
will also be invited to regular team meetings and clinical supervision in
which you will be encouraged to take part.

by a team of commercial partners and experienced General Practitioners with a
clear vision of providing services that would contribute to and benefit the

known as Hampshire Doctors on Call) runs Integrated Urgent Care, which evolved
from the former Out of Hours model, for localities in Hampshire and the Isle of
Wight. All of the core services which look after patients with an urgent healthcare
need are joined up together, and PHL work closely with system partners such as South
Central Ambulance Service. The services we operate include call handling,
telephone clinical assessment, out of hours home visiting services and some face-to-face
Primary Care appointments services in Hampshire. In 2021 a newly formed
Healthcare Division was created and incorporates PHLs Urgent Care, Primary
Care and Youla Care Concierge Services. The new division combines PHL's
healthcare operations into one team and utilises internal and external

- Work autonomously and be accountable for your own professional actions.
- Assess undifferentiated patients between the ages of 0-16 yearswith a range of acute, non-acute and chronic medical conditions.
- Throughremote telephone and video consultations clinically assess a wide range of paediatric patients through history taking,identifying underlying causes andcarry through managementand treatment plans usingthe support of the Healthier Together Pathways and NICE guidelines.
- Facilitating management plansand chose the dispositionthatwould be the most appropriate to the patients needs.
- Where remote prescribing is felt necessary thenappropriatepatientscan be forwardedto a medical or non-medicalprescriberfor furtherassessment.
- Refer and facilitate patient pathways through appropriate referrals to primary, community, secondary and tertiary care for patients ensuring direct referral if urgent care required to appropriate specialty.
- Recognise the need and initiate emergency treatment, either referring direct to ED or using the 999 services.
- To ensurehome managed patientshave robust safely netting advice and access to further information for worsening symptoms via the 'Healthier Together' app/ website or NHS online.
- To be professionally and legally responsible and accountable for all aspects of clinical decision making within your clinical competence
- The post holder will promote a robust culture of safeguarding practice, and ensure they work closely with the safeguarding hub and childrens services.
- Take a person-centred approach to service delivery, based on clear understanding and experience of the modes for service delivery.
- Maintain and develop clinical competence and knowledge of care delivery within urgent care setting using both formal and informal training methods.
- To be responsible and accountable for legible and contemporaneous clinical records
