Job summary
Law Medical is seeking to recruit an Advanced Nurse Practitioner to join its multidisciplinary primary care team. The role will involve delivering high-quality, patient-centred clinical care, supporting the assessment, diagnosis, and management of a wide range of acute and long-term conditions, while working collaboratively with GPs and other healthcare professionals.
The Advanced Nurse Practitioner will work autonomously within their scope of practice, contributing to clinical decision-making, prescribing, and care planning. The post holder will also play an active role in supporting the wider multidisciplinary team and contributing to the ongoing development and objectives of the practice.
Main duties of the job
As an Advanced Nurse Practitioner, you will work autonomously within your scope of practice to deliver high-quality, patient-centred care. Key responsibilities include managing acute and chronic conditions, conducting health assessments, running specialist clinics, prescribing medications, and promoting health and disease prevention. You will also contribute to clinical policy development, support safeguarding, and play an active role in clinical audits and service improvement across the practice.

Clinical
Responsibilities:
In accordance with the practice timetable, the ANP will work
autonomously, delivering care within their scope of practice to the patient
population. Contributing towards clinical areas such as health promotion,
chronic disease management, health prevention, well-women and well man clinics
as well as supporting the management team in the reviewing of clinical policy
and procedure.

Manage patients presenting with a range of acute
and chronic medical conditions, providing subject matter expert advice to
patients

Implement and evaluate individual specialised
treatment plans for chronic disease patients

Identify, manage and support patients as risk of
developing long-term conditions, preventing adverse effects on the patients
health

Provide advanced, specialist nursing care to
patients as required in accordance with clinical based evidence, NICE and the
NSF

Provide wound care (ulcer /Doppler etc.) to
patients

Provide specialist clinics such as ear
syringing, contraception etc.

Undertake the collection of pathological
specimens

Provide travel medicine services

Request pathology services as necessary

Process and interpret pathology and other test
results as required
