Job summary

Following the release of ARRS GP information and funding we are looking for newly qualified (within 2 years and not having a substantive post), enthusiastic forward-thinking GP(s) to join South Southall PCN on a fixed term salaried contract, preferably with an immediate start date.

The postholder's role will primarily involve patient-facing clinical work supporting the PCN GP practices alongside a focus on PCN initiatives including novel, innovative at-scale work and opportunities. Additionally, they will offer support to the GP Enhanced Access Hub within the PCN, providing enhanced services to the registered population.

South Southall PCN comprises 8 GP practices, serving a population of 65,000 patients. We pride ourselves on our well-established and collaborative inter-practice relationships, with a shared goal of improving the health and outcomes of our patients within the local community.

Main duties of the job

Clinical responsibilities

1.Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team

2.Implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis

3.Clinically examine and assess patient needs including history taking, assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and onward refer where appropriate

4.Deliver high-quality healthcare to care home residents who often have complex medical needs, conducting thorough initial assessments upon admission to the care home

5.Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice

6.Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice

7.Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health Implement and participate in vaccination and immunisation programmes for both adults and children

8.Support the delivery of advanced care plans
