Job summary
The main functions are to deliver structured group education to people diagnosed with type 2 diabetes using the DESMOND training program and to provide cover for colleagues during periods of planned and unplanned leave.
The post holder is also expected to support people to make informed decisions with regards to lifestyle choices and signpost those receiving education to appropriate services.
Main duties of the job
Deliver
structured group education to people diagnosed with type 2 diabetes using the
DESMOND training programme via group sessions.
Support
people to make informed decisions with regards to lifestyle choices.
Signpost
those receiving education to appropriate services.
Ensure
attendance and outcome data is supplied to administrative
staff in a timely manner.
Ensure
attendees complete evaluations of the sessions and the forms are supplied to
administrative
staff in a timely manner.
Assist
in the on-going development and delivery of sessions across Coventry and Rugby,
including reviewing the modules that are delivered and locations used and feedback to manager.
Ensure
training equipment is available at all sessions and is well maintained.
Ensure
adequate stocks of patient literature are maintained.
Facilitate
observations of the sessions by new trainers and other HCPs.
Support
new trainers to deliver sessions following attendance at training courses.
Attended
on-going DESMOND training courses as required.
Plan
and organise required Quality Development sessions as required to obtain and
maintain DESMOND accreditation Actively participate in keeping up to date,
attend relevant learning activities and organisation mandatory and statutory
training.
Ensure
that all organisational standards are maintained and monitored to improve the
quality of care to all whom come into contact with the service.
