dedicated individual to fill the role of General Practice Nurse with Skyward
PCN.
In this position, you will support us with achieving our
goals, while also contributing to the overall success of our organisation.
The post holder is expected to have competence in long-term
condition management, managing long-term conditions and caring for elderly,
frail, housebound, or care home patients. They should also be capable of managing raised blood pressure, Cholesterol,
Diabetes, Asthma, COPD care and have EMIS experience.
GP practice experience is also essential,
including supporting and managing a wider practice team - HCAs, Nursing
Associates, Nurses.
The candidate must also have current
UK professional registration.
They must have their own transport and ability to drive.

Long-Term Condition Management and Elderly Care
Lead and manage long-term conditions such as hypertension, diabetes, asthma, COPD, and cholesterol management.
Provide care for elderly, frail, and housebound patients, including those in care homes.
Conduct home visits as required.
Support quality improvement and assurance initiatives within the PCN.
General Practice Nursing Duties
Conduct chronic disease management within the scope of expertise.
Provide immunisations, including:
Routine influenza, pneumococcal, shingles vaccinations.
Infant immunisation and travel health care vaccinations.
Perform cervical smears, pregnancy tests, and contraceptive advice.
Assist GPs with minor surgeries and coil fittings if required.
Carry out ECGs, venepuncture, urinalysis, and wound care including ulcer management and suture removal.
Review and action pathology tests such as urine culture and swabs where appropriate.
Chaperone and assist patients being examined by other clinicians.
Leadership & Team Support
Mentor and supervise any new-to-practice nurses, nursing associates, and HCAs.
Work collaboratively with the wider practice team to enhance patient care.
Promote public health and screening programs, including immunisations and cervical screening.
Integrate population health management approaches to reduce health inequalities.
Clinical Governance and Compliance
Maintain accurate and up-to-date patient records.
Ensure compliance with clinical protocols, including prescribing audits and reviews if qualified.
Adhere to NMC revalidation requirements and practice within the NMC Code of Conduct.
Follow infection control policies and ensure safe handling of specimens.
Contribute to audits and service evaluations.
Support the effective use of NHS resources and practice policies.
Practice and Housekeeping Duties
Ensure the cleanliness and tidiness of nursing and treatment rooms.
Maintain and clean clinical equipment.
Monitor and order medical stock, drugs, and vaccinations.
Attend and contribute to practice meetings and training sessions.
Health and Safety Responsibilities
Comply with national infection control standards and health and safety guidelines.
