Job summary
Location: Skyward PCN, Longford Medical Centre
Working patter: Part time (Tuesday, Wednesday, Thursday)
As
a PA you will be acting within your professional boundaries, providing care for
patients from initial history taking, clinical assessment, diagnosis, treatment
and evaluation of their care under the supervision of a GP. You will demonstrate safe clinical
decision-making and expert care for patients and work with the
multi-disciplinary general practice team to support the delivery of clinical
care and meet the needs of patients.
Mentorship and supervision will
be provided by designated supervising GP. The level and type of supervision
will be dependent on the post holders skills and knowledge and determined by
the organisations clinical governance arrangements. You will follow
recommended annual appraisals and continuing professional development set out
by the Faculty of Physician Associates (FPA) and be registered on the Managed
Voluntary Register (MVR).
Main duties of the job
- Provide a first point of contact for patients presenting with undifferentiated, undiagnosed problems by utilising history taking, physical examination and clinical decision-making skills to establish a working diagnosis and management plan in partnership with the patient.
- Deliver clinical care according to NSF, NICE guidelines and evidence-based care
- Support the completion on clinical workflow- laboratory reports, on-line consultations, referrals and clinical documentation as advised.
- Direct referrals appropriately and within locally agreed pathways, guidance and protocols
- Liaise closely with the GP in determining prescribing needs.
- Ensure effective communication skills and be able to advise and provide information to patients, carers and their families where appropriate.
- Identify and take appropriate actions to address risks to the patient, this will include;
- Identification of emergency care needs requiring dispatch of a 999 ambulance
- Safeguarding children and protection of vulnerable adults issues
- Apply infection-control measures within the practices according to local and national guidelines.
- Take responsibility and being accountable for managing own workload, risk assessment and management, acting within professional codes and guidelines, and local policies, procedures and guidelines and referring appropriately to the clinical team leader for advice and support.
- Undertake other duties when require, such as audits or administrative tasks.
